About this file
This is a Conflict of Interest Affidavit form issued by the State of Maryland in July 2024, applicable to offerors, contractors, consultants, and subcontractors bidding on the COVID PCR Testing contract at Spring Grove Hospital Center. The affidavit requires all participants in the procurement process to warrant that no conflicts of interest exist that would impair their objectivity or provide unfair competitive advantage in performing contract work. Offerors must disclose any relevant facts or circumstances that give rise to actual or potential conflicts of interest, providing detailed explanations and supporting documentation as necessary.
The affidavit establishes an ongoing obligation for contractors to immediately notify the procurement officer in writing if any conflict of interest arises after execution of the document, including a description of actions taken or proposed to mitigate or neutralize the conflict. The document must be signed under penalty of perjury by an authorized representative and submitted with the bid or proposal. Compliance with this conflict of interest disclosure requirement is mandatory for all bidders seeking to participate in this Maryland procurement opportunity.
