About this file
This is a Section 504 Compliance Assurance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to the Merchant Credit Card Services procurement. The document establishes that contractors must comply with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations, guidelines, and interpretations. The assurance is required as a condition for obtaining contracts and is binding on the contractor, its successors, transferees, and assignees. Contractors must certify their compliance status by completing the form with authorized signatory information, including company name, address, and contact details.
The form provides two compliance options based on contractor size: contractors employing fewer than 15 persons may follow a simplified compliance pathway with a potential exception allowing referral to accessible alternative providers if facility modifications would constitute significant alterations, while contractors employing 15 or more persons must designate a specific Section 504 Coordinator to oversee compliance efforts in accordance with 45 C.F.R. 84.7(a). The assurance requires certification that submitted information is complete and accurate, with signature and date from an authorized official. This compliance requirement ensures that merchants and service providers under the Merchant Credit Card Services contract maintain non-discriminatory practices and accessibility standards for individuals with disabilities throughout the contract period.
