About this file

This document is an Assurance of Compliance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, specifically related to Section 504 of the Rehabilitation Act of 1973. The form requires contractors to affirm their commitment to complying with disability non-discrimination regulations, with specific requirements depending on the number of employees. Contractors must either confirm they employ fewer than 15 persons or, if employing 15 or more persons, designate a specific person to coordinate efforts to comply with DHHS regulations.

The form provides a template for contractors to certify their compliance, including fields for the name of a 504 compliance coordinator, contractor name, address, and signature of an authorized official. An important exception note indicates that organizations with fewer than 15 employees who cannot make facility accessibility accommodations may alternatively refer disabled persons seeking services to other accessible providers. While this document is part of a broader procurement process for Merchant Credit Card Services, it specifically focuses on ensuring equal access and non-discrimination compliance for potential contractors.
