About this file
This document is an attachment form for OregonBuys Bid S-62900-00015613 for a Type 2 Restricted Category Turbine Powered Helicopter contract. Attachment H requires proposers to submit a Helicopter Pilot Summary for each primary base location, including information for both primary and relief pilots designated to perform services under the contract. Proposers must complete detailed pilot qualification information including FAA certificate numbers, certificate types, airmen ratings, FAA medical certificate details, and FAA Part 133 and Part 137 compliance status. The form requires documentation of specific operational experience, including two fire seasons of bucket-loading and long-line operations from hover positions and aerial delivery of firefighters. Proposers must provide current USFS/OAS Carding and copies of FAA Pilot Certificates for all designated pilots.
Attachment H-1 establishes a desirable features evaluation component worth a maximum of 80 points for primary and relief pilots only. Scoring is based on two criteria worth up to 20 points each: years of aerial wildland fire suppression long-line and bucket work experience beyond the two-year minimum requirement (1 point per year, maximum 20 points), and months of experience flying in the Southern, Central, or Eastern Oregon geographic area (1 point per month, maximum 20 points). Only the two pilots listed on Attachment H-1 will be evaluated; proposers should not submit this attachment for additional pilots. Proposers must clearly identify pilot names as submitted on Attachment H and calculate experience by subtracting the two-year minimum requirement from total years of service.
