About this file
This document is a Partial Waiver of Lien form issued by Macomb Township, Michigan, designed to be submitted by contractors and subcontractors with periodic progress payment requests. The form provides a legal mechanism for contractors to waive lien rights to the extent of payment received for labor, materials, services, fixtures, apparatus, or machinery furnished for a specific project. In this case, the form is associated with an insulation project at Fire Stations 3 and 4, where the contractor will furnish supplies, materials, delivery, installation, warranties, and related services for expanding foam insulation.
The form includes detailed instructions for completion, specifying how to fill out sections such as the contracting parties, work description, contract details, and payment amount. While the specific monetary value is not filled in, the document indicates that proposals of $50,000 or more will require US Treasury Listed Company Bonds. The waiver is specific to Macomb Township in Macomb County, Michigan, and serves as a protective legal document to prevent duplicate claims or liens on the project by acknowledging receipt of payment for work completed.
