About this file
This is an evaluation form (Attachment G) used by the Florida Department of Elder Affairs (DOEA) for assessing past performance references as part of solicitation 24-RFP-001-JT for the Senior Community Service Employment Program (SCSEP). The form outlines a structured interview process where DOEA representatives interview client references chosen at their discretion to evaluate previous contractor performance. The evaluation covers six key performance areas using a point-based scoring system.
The evaluation criteria include adherence to contract terms, quality of work, adequacy of personnel (quantity, experience, and profession), meeting performance expectations, and history of corrective actions. Each category is scored on a 5-point scale (Excellent=5 points to Poor=1 point), except for the corrective actions category which uses a different scale (No=5 points, Resolved=2 points, Unresolved=0 points). The final score is calculated by totaling all points and dividing by 5 to obtain an average score.
