About this file
This is an Evaluation of Past Performance form (Attachment G) from the Florida Department of Elder Affairs (DOEA) Solicitation No. 24-RFP-001-JT. The document is a standardized reference check form used by DOEA to evaluate contractors' past performance through client references. The form includes fields for basic information including the respondent's name, reference name, person interviewed, interviewer, and date of interview.
The evaluation consists of six questions scored on a point system, covering contract adherence, quality of work, personnel adequacy, performance expectations, and corrective actions. Questions 2-5 use a 5-point scale (Excellent=5, Above Satisfactory=4, Satisfactory=3, Fair=2, Poor=1), while question 6 regarding corrective actions uses a different scale (No=5 points, Yes resolved=2 points, Yes unresolved=0 points). The total score is divided by 5 to calculate an average score, which appears to be used as part of DOEA's contractor evaluation process.
