About this file
This is a Responsibility Inquiry form (Attachment E) for RFP 4056 issued by the City of Springfield, Oregon, for Construction Manager/General Contractor (CM/GC) services related to the City Hall Deferred Maintenance and Quad Redesign Project located at 225 5th Street, Springfield, Oregon. The form serves as a pre-award assessment tool to evaluate the financial and operational capability of proposers prior to contract execution. The City of Springfield will determine proposer responsibility and may request additional documentation including profit-and-loss history, balance statements, cash flow information, assets-to-liabilities ratios, bonding capacity, insurance information, equipment and facility capabilities, personnel qualifications, and performance records under previous contracts. Failure to promptly provide requested information or demonstrate responsibility may result in a finding of non-responsibility and rejection of the proposal.
The Responsibility Inquiry form requires proposers to attest to their financial resources, materials, equipment, facilities, and personnel capabilities to meet all contractual responsibilities. Proposers must disclose the number of similar contracts completed within the last five years that remained within time and budget constraints without claims, as well as any contracts that failed to meet these standards. The form also requires disclosure of any criminal or civil charges, indictments, or convictions involving the proposer, partners holding 10% or more ownership, or major subcontractors receiving 10% or more of the contract amount related to public contracting, antitrust violations, or fraud. Additionally, proposers must disclose contract terminations for default within the last ten years, pending lawsuits, outstanding judgments, financial distress, bankruptcy filings, and confirmation of all required licenses, insurance, registrations, and legal authorization to conduct business in the State of Oregon. All responses must be certified as complete and accurate by an authorized representative.
