About this file

This is a Past Performance Questionnaire template for Solicitation No. 1305M426Q0048, NWS Southern Region WFO Emergency Power Generator Replacement (Multiple Sites). The questionnaire is to be completed by references providing performance feedback on offerors and must be submitted to the Contracting Officer Noelle Albert at noelle.albert@noaa.gov no later than 12:00 PM Eastern Standard Time on September 4, 2026. The email subject line must clearly reference the solicitation number and indicate it is a Past Performance Questionnaire Submission.

The questionnaire requires references to provide objective assessments across seven performance elements: Quality of Product or Service, Schedule, Cost Control, Business Relations, Management of Personnel, Utilization of Small Business, and Additional Comments. References must rate contractor performance using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory) for each element, with substantive comments required to support ratings. The form collects reference information (name, title, organization, contact details, and role) and project information (contractor name, contract number, project title, procurement vehicle type, contract type, award date, dollar value, and period of performance). Cost Control ratings are not applicable for Firm Fixed Price contracts, and Small Business Utilization assessments are not required for small business contractors. References should provide candid, detailed responses to assist in the vendor evaluation process, with the possibility of follow-up phone calls to clarify information.
