About this file
This is a Mandatory Site Visit Validation Form for RFB 758 2600000003 issued by the Robert E. Spiller Bowling Green Veterans Center (RESBGVC) in Bowling Green, Kentucky. Bidders are required to conduct an in-person inspection of the facility located at 1111 Veterans Way, Bowling Green, Kentucky, prior to submitting their bid response. The site visit must be arranged by contacting Will Humphrey at Will.Humphrey@ky.gov or (270) 438-9671. Upon completion of the facility tour, bidders must obtain the signature of an authorized RESBGVC representative on this form and submit the signed document with their bid to be considered for award. Failure to complete the mandatory site visit and return the signed validation form with the bid response will result in disqualification and render the bid non-responsive.
Bidders are responsible for inspecting all areas of the facility and satisfying themselves regarding general and local conditions that may affect contract performance costs. Importantly, no pricing information shall be disclosed to agency representatives during the site visit, and any violation of this requirement will render the bid non-responsive. The form must include signatures from both the authorized RESBGVC representative and the company representative who toured the facility, along with printed names, company name, and dates. Site inspection is the bidder's sole opportunity to verify actual dimensions and conditions; failure to inspect does not constitute grounds for post-award claims.
