About this file

This document is a Required Affidavit attachment for the Commonwealth of Kentucky establishing mandatory attestations for bidders, offerors, and contractors seeking state contract awards. The affidavit is effective for one year from the date of execution and requires an authorized representative of the contracting party to complete the attestation, obtain notarization, and submit it to the Commonwealth in accordance with KRS 45A.110 and 45A.115. No specific products, services, quantities, response dates, award dates, or contract terms are detailed in this document, as it functions as a procedural requirement applicable across multiple procurement opportunities rather than addressing a specific solicitation.

The affidavit requires contractors to attest under penalty of perjury that they have not knowingly violated Kentucky campaign finance laws and that contract award will not violate any campaign finance law provision. The document includes three separate affidavit forms: a general campaign finance attestation, a resident bidder status affidavit requiring proof of Kentucky business authorization and one year of tax filings, unemployment insurance contributions, and workers' compensation coverage, and a qualified bidder status affidavit for nonprofit corporations serving individuals with severe disabilities. The Commonwealth reserves the right to request updated affidavits and supporting documentation at any time, with failure to comply potentially resulting in bidder disqualification or contract termination. All affidavits must be notarized and include the notary's seal and commission expiration date.
