About this file
The document is a Mandatory Site Visit Validation Form for the Robert E. Spiller Bowling Green Veterans Center (RESBGVC) located at 1111 Veterans Way in Bowling Green, Kentucky, associated with a Request for Bid (RFB 758 2600000003). The form requires potential bidders to conduct a mandatory site visit and obtain an authorized representative's signature to be considered for contract award. Bidders must inspect the facility where services are to be performed, understand local conditions that may affect contract performance, and verify the actual size of specified areas. The site visit is critical, as failure to tour the grounds and obtain the required signature will result in bid disqualification.
The form emphasizes strict requirements for the site visit, including a prohibition on discussing or revealing any pricing information during the facility tour. Interested bidders must contact Will Humphrey at Will.Humphrey@ky.gov or (270) 438-9671 to arrange the site visit. The document underscores that no claims regarding site conditions will be considered after contract award, placing the responsibility on bidders to thoroughly inspect and understand the facility prior to submitting their bid. This validation form is a mandatory component of the bid response and must be completed and submitted with the bid package to be considered responsive.
