About this file
The document is a set of three required affidavits for bidders, offerors, and contractors in the Commonwealth of Kentucky, specifically related to state procurement processes. These affidavits include: 1) a general campaign finance compliance affidavit, 2) a resident bidder status affidavit, and 3) a qualified bidder status affidavit. Each form requires bidders to swear under penalty of perjury that they meet specific legal requirements for doing business with the state, such as being authorized to transact business in Kentucky, having filed corporate taxes, made unemployment insurance payments, and maintained workers' compensation policies.
The affidavits outline specific compliance requirements for different bidder classifications, including provisions for nonprofit corporations, agencies serving individuals with severe disabilities, and general business entities. The documents emphasize that the bidding agency reserves the right to request documentation supporting claims of status and that failure to provide such documentation may result in disqualification or contract termination. The forms are designed to ensure transparency, legal compliance, and fair competition in state procurement processes, with each affidavit being valid for one year from the date of execution.
