About this file
This document is a Deliverables and Reports table outlining submission requirements for a VA Community Based Outpatient Clinic (CBOC) contract. The table details what documentation must be submitted, when it must be submitted, who it must be submitted to (primarily CO/CS or COR), and the corresponding compliance references.
Key deliverables include: past performance references, healthcare operations experience, facility information, management approach documentation, continuity of care plans, staff certifications and licenses, subcontracting plan, VOSB/SDVOSB verification, pricing schedules, background checks, and various operational reports. Post-award requirements include cyber security certifications, PIV cards, patient safety reporting, Joint Commission accreditation, credentialing documentation, workload reports, and quarterly TJC standards audits. The document emphasizes that contractors remain responsible for all items listed in the Performance Work Statement, even if not explicitly included in this deliverables table.
