About this file
This document is an attachment (D.5) titled "Enrollment Priority Groups" that outlines the VA's health care benefits enrollment system and priority groups. It is form IB 10-441 dated April 2018.
The document details eight priority groups that determine Veterans' enrollment eligibility for VA health care benefits. Priority Group 1 includes Veterans with 50%+ service-connected disabilities, those deemed unemployable due to service conditions, and Medal of Honor recipients. Groups 2-5 cover Veterans with varying disability ratings (30-40%, 10-20%), former POWs, Purple Heart recipients, those requiring aid/attendance, and Veterans with specific income limitations. Priority Group 6 includes Veterans exposed to specific service conditions (Vietnam, Gulf War, Camp Lejeune) and combat operations veterans. Groups 7-8 are based on household income levels and willingness to pay copays. The document also details subpriorities a-g within Group 8 that further define enrollment eligibility based on enrollment dates and income thresholds. This appears to be a reference document for VA healthcare enrollment policies rather than containing specific requirements for the Holly Springs CBOC services contract.
