About this file

This is Addendum No. 1 to a procurement solicitation issued by the Commonwealth of Kentucky for WSH and Nursing Facility Laundry and Linen Service. The addendum clarifies key contract requirements and answers vendor questions regarding service delivery and bid submission requirements. The expected contract start date is October 15, 2026. The contract requires bidders to submit unit costs for ten line items based on estimated weekly usage quantities provided on pages 11-12 of the original solicitation. Bidders must provide a per-item unit price that encompasses all required services, equipment, and materials, including carts, hampers, bags, stands, liners, initial and emergency inventory, transportation, reporting, and culture testing, with no separate reimbursable charges permitted. A one-week transition meeting and three-day transition inventory requirement are mandated prior to service commencement.

The addendum explicitly prohibits the use of subcontractors; all experience and service delivery must be provided by the prime contractor only. Vendors competing on years of laundry and linen service experience can earn a maximum of five points, with documentation such as Articles of Incorporation, Secretary of State Registration, or business financial records required at bid submission; vendors failing to provide documentation will receive zero points and may be deemed non-responsive and ineligible for award. Bid evaluation includes a 95-point price calculation based on total evaluated weekly pricing. The prime contractor's required full-time authorized service representative may be located outside Hopkinsville provided the representative is dedicated to the account and can fulfill all onsite and service obligations. Loss percentage is not factored into line-item bids, and all costs necessary to fulfill contract terms must be inclusive in pricing with no reimbursable expenses allowed.

Attachment D Addendum No. 1

BIDDER SHALL CONFORM TO THE FOLLOWING CHANGES AS SAME SHALL

BECOME BINDING UPON THE CONTRACT TO BE ISSUED IN RESPONSE TO THIS

SOLICITATION.

Reason for change: To answer potential vendor questions and omit Section 32 in Attachment A.

Vendor Questions Commonwealth Response

Please confirm whether a prime contractor may receive the five (5) experience points based on the years of laundry/linen experience of a proposed subcontractor that will perform the specialized laundry operations. If permitted, please identify the documentation required and how the years should be reported.

Subcontractors are not permitted for this contract. All experience must be from the prime vendor. Vendors with the highest number of years of experience as a provider of laundry and linen services will receive a maximum of five (5) points.

Vendors should provide documentation with their bid submission. Verification documentation may include such items as Articles of Incorporation, Secretary of State Registration, Certificate of Assumed Name filed with the County Clerk, Business or Financial Records showing proof of purchase of business/company.

Vendors not providing documentation with their bid submission will receive zero (0) points for this criteria and may be deemed non-responsive and ineligible for award.

Please confirm whether the proposed laundry subcontractor must be identified at bid submission and, if so, what subcontractor information/documents must be included.

Subcontractors are not permitted for this contract.

Please confirm where the required fixed flat percentage for loss is to be entered in the Procurement response and whether one loss percentage applies to all ten line items.

Percentage loss is not factored into the line-item bid.

Please confirm whether the bidder's per-item unit prices are expected to include all required carts, hampers, bags, stands, liners, initial inventory, emergency inventory, transportation, reporting, culture testing, and other required service costs, with no separate reimbursable charges.

All costs required to fulfil contract terms must be inclusive in pricing. There are no reimbursable costs.

Please confirm that the 95-point price calculation is based on each bidder's total evaluated weekly price using the estimated weekly quantities shown on pages 11-12, with the bidder entering a unit cost for each of the ten line items.

See Section 2- B Service Requirements: Usage is only an estimation and may vary depending on census along with other factors. The average estimated weekly usage is how the bidder shall bid to arrive at a cost per item. The bid price shall be evaluated on this usage.

Please clarify the expected contract start date or anticipated service commencement date so bidders can plan the one-week transition meeting and three-day transition inventory requirement.

Expected contract start date is 10/15/26.

Please confirm whether the prime contractor's required full-time authorized service representative may be based outside Hopkinsville provided the representative is dedicated to the account and can meet all required onsite/service obligations.

Service representative may be based outside Hopkinsville provided the representative is dedicated to the account and can meet all required onsite/service obligations.
