About this file

This is a Substitution Request Form associated with an HVAC replacement project at Port St. John Community Center in Cocoa, Florida. The form serves as the mechanism by which contractors may propose equivalent materials or products as substitutions to those specified in the contract documents prepared by TLC Engineering Solutions (dated 12/17/2025). Contractors must submit detailed information for any proposed substitutions, including item number, item description, manufacturer name, model number, packaging quantities, and supporting documentation such as cut sheets and data sheets. The project involves furnishing all labor, materials, tools, equipment, and transportation necessary for the HVAC replacement at 6650 Corto Road, Cocoa, FL 32927, with all work performed in conformance with the contract drawings and applicable federal, state, and local regulations.

The form explicitly states that submission of proposed equivalents is entirely at the contractor's own risk and warns that any proposed equivalent or exception deemed to constitute a material deviation from specifications will result in mandatory bid rejection. The form cannot be used to attach conditions, limitations, or other provisos to the bid. Contractors must provide their company name, address, telephone, fax information, and authorized signature. If no exceptions are taken on the form, the contractor is bound to supply all materials exactly as specified in the contract documents.
