About this file

This is a Price Sheet attachment for a Government Crime Insurance Invitation to Bid (ITB No. 27-84131514) issued by the Florida Department of Management Services on behalf of multiple state agencies and universities. The solicitation seeks a single broker to provide comprehensive crime insurance coverage for state entities including the Department of Education, Department of Revenue, Department of State, Florida universities (UF, FSU, USF, FAMU, etc.), and various other state agencies. The coverage includes employee theft blanket protection for 95,753 total employees across participating agencies, excess employee theft coverage, faithful performance of duty bonds, forgery and alteration coverage, inside money and securities coverage up to $50.7 million, inside robbery coverage up to $1.5 million, and outside premises coverage up to $52.1 million. Brokers may submit pricing for one to five consecutive policy periods beginning January 1, 2027 through January 1, 2032, with each policy period covering a 12-month term.

Pricing must be submitted on the locked Excel Price Sheet with all premiums based on commission-based broker compensation rather than broker fees, with pricing evaluated on an equal basis across all bidders. Brokers must provide annual premiums for each insuring agreement, identify any statutorily required assessments as a percentage of premium subtotal, itemize additional fees such as policy or inspection fees, and ensure all coverage premiums assessed at policy inception remain fixed for the full policy period without changes. The Price Sheet automatically calculates a Discounted Premium using a 2.30% inflation rate from the Survey of Professional Forecasters and a Calculated Premium representing the average of discounted premiums across the bid period for scoring purposes. Surplus lines tax fees are prohibited under Florida Statutes, and terrorism risk insurance pricing should not be included. No alterations to the Price Sheet specifications are permitted, and the completed file must be uploaded in its original Microsoft Excel format, not converted to PDF or other formats.
