About this file

This is a Past Performance Questionnaire template for the Bureau of Engraving and Printing (BEP) solicitation 2031ZA26Q00131 for Uniform Dry Cleaning Services for the BEP Police in Washington, DC. Offerors must submit questionnaires to a minimum of three assessors whose services were relevant to the requirement—defined as similar in size, scope, and complexity to the BEP's needs. The assessors must be current or historical personnel or organizations for which the offeror provided services within the three years preceding the Request for Quotation issuance date. Completed questionnaires must be submitted directly by the assessors (not the offeror) to marico.sellers@treasury.gov no later than 12:00 PM Eastern on Monday, August 31, 2026.

The questionnaire requires assessors to validate contract information provided by the offeror in Section II, including contractor name, contract number, contract type, contract value, period of performance, and project description. Assessors must then complete their evaluation in Section V, rating the contractor's performance across six categories—Customer Satisfaction, Schedule Performance, Management Performance/Program Management, Quality, and Cost—using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory) plus a Not Applicable option. Assessors must identify their role in contract administration (such as Procuring Contracting Officer, Administrative Contracting Officer, Contract Specialist, Contracting Officer's Representative, or other relevant position) and provide their period of involvement. The questionnaire concludes with an assessment of overall contractor performance and whether the assessor would recommend awarding another contract to the contractor.
