About this file

This is an Administrative Procedure document (NNFD-004, Revision 11) for the Nonreactor Nuclear Facilities Division (NNFD) that establishes work control requirements for operations, maintenance, and service activities within Oak Ridge National Laboratory (ORNL) nonreactor nuclear facilities.

The procedure implements a graded approach to work management across all work types, including new construction, corrective maintenance, preventive maintenance, predictive maintenance, calibrations, and subcontracted work. The document defines three categories of work: Emergency Work (requiring immediate FMGL and Division Director approval), Urgent Work (requiring FMGL, Division Director, and SME concurrence), and Non-Emergency Work (following standard screening and planning processes). All work in NNFD nuclear facilities is classified as Grade 1 (subdivided into categories A-H) based on risk level and documentation requirements. The work control process includes seven primary phases: Work Identification (submission via electronic service request or CMMS), Work Screening (validation of need, grading, priority, and status assignment by FMGL or designee), Work Planning (development of detailed work packages with hazard evaluations and prerequisite conditions), Work Execution (actual performance with pre-job briefings and daily progress documentation), Work Package/Plan Changes (revision control with USQ determinations for intent changes), Post-Maintenance Testing (verification of component operability following maintenance), and Work Closure (completion checklist and feedback documentation). Key procedural requirements include assignment of qualified Task Leaders per Qualification Card standards, involvement of Subject Matter Experts for hazard identification and control, USQD screening for nuclear facilities, compliance with configuration management and modification control procedures, and maintenance of complete work package records forwarded to the Document Control Center. The procedure references DOE Order 433.1, Standards-Based Management System (SBMS) guidance, and facility-specific procedures including NNFD-001 (Document Development), NNFD-002 (Change Control), NNFD-018 (Pre-Job Briefs), and NNFD-023 (Balance of Plant Modifications). Appendices provide definitions, work package formatting guidelines, and post-maintenance testing matrices for various equipment types and maintenance activities.

1. INTRODUCTION

1.1 Purpose

The purpose of this procedure is to ensure a uniform graded approach to work management and establish the requirements and instructions for planning, scheduling, initiating, controlling, and documenting actions pertaining to work control within the Nonreactor Nuclear Facilities Division (NNFD).

1.2 Scope

The requirements, guidance and references in this procedure apply to operations (other than operational evolutions covered by a procedure approved in accordance with NNFD-001, Document Development, Review, and Approval), maintenance, and service work (e.g. new, corrective, preventative maintenance, predictive maintenance, calibrations, new construction, subcontract) performed within or affecting the NNFD facilities. This document establishes the controls and methodology for the administration of work related activities within or affecting all NNFD facilities.

This procedure provides a maintenance work control program that addresses identification of needed work, planning and preparation for work, evaluation of work hazards, development of hazard controls, establishment of conditions to perform work, return of system to operations (system control), and review of completed work records and document control.

Facility-specific operating procedures may be used in lieu of test plans, work control plans and packages.

No primary user is identified for this procedure. Since multiple users are responsible for performing portions of this procedure, user responsibilities are identified throughout this procedure.

1.3 Work Management

The process defined in this procedure is designed to implement work management that:

• Ensures the appropriate level of rigor is applied to work based on a graded approach.

• Provides a NNFD common approach such that work management is accomplished using the same structure and methodology in all areas.

• Provides the directions, tools, requirements, and references to create a total work management system.

1.4 Work Management Processes

EMERGENCY WORK

When the need for emergency work (as defined in Appendix A, Definitions/Acronyms) is recognized, inform the Facility Management Group lead (FMGL) and follow Oak Ridge National Laboratory (ORNL) and NNFD notification requirements as applicable. Execute the work and using a blank Work Log and Pre-Job Brief, document the steps taken and approval of work by the FMGL and Division Director, or designee. After the hazard is controlled, submit a Work Request and scan and attach the forms to the Work Order for record.

URGENT WORK

When the need for urgent work (as defined in Appendix A, Definitions/Acronyms) is recognized, inform the Facility Management Group Lead (FMGL). Using a blank Work Log, document the steps to be taken including review and approval of work by the FMGL, Division Director or designee, and concurrence by Subject Matter Experts (SMEs) as determined by the FMGL along with the conduction of a Pre-Job Brief per NNFD-018. After the work is completed, submit a Work Request, scan the forms, and attach the forms to the Work Order for record. Compile Work Order and associated forms into Work Package. Obtain FMGL approval to close Work Package and document in the Work Log Section of NNFD-FRM-058, Work Package Forms. Forward the completed Work Package to Document Control Center (DCC) for record retention.

NON-EMERGENCY WORK

Identify Need - The need to perform a Task is identified by any staff member through the Facility Service Center electronic service request system, or directly into the Computerized Maintenance Management System (CMMS).

NOTE: For operations activities which will utilize an Operations Work Plan, Work Order creation and screening is not required

Validate Need – The electronic service request or Work Order is reviewed by the NNFD Facility Management Group Lead or designee and screened to validate the need for the work and establish the grade, priority and status. The work screener then ensures that the Work Order identifies the asset which is applicable to the activity. All NNFD Configuration Items have assets established in the CMMS. Other equipment may have assets identified in the CMMS as well. The work screener is required to link the work order to the asset when an asset is identified in CMMS. Attaching work orders to assets is necessary in order to maintain system and component maintenance history.

The grading criteria identified in SBMS, (Grade 1(A-H), Grade 2, Grade 3, Grade 4) provides the necessary graded approach that evaluates both the level of risk and rigor required to ensure that the proper controls are in place to conduct the work safely and effectively. All work performed in NNFD Nuclear Facilities is considered to be Grade 1 work and as such are assigned Grade 1 (A-H).

The work is then assigned to a task leader for planning and scheduling.

1.4 Work Management Processes (cont)

Identify Resources and Requirements – Consists of determining scope, requirements, variables and constraints associated with the requested work.

Develop Work Package - A work package is developed by the work package/plan author to provide the proper prerequisite conditions, work instructions, post-maintenance testing, and data records necessary to resolve the requested work. Specific NNFD direction for Work Plan formatting and comment resolution is contained in Appendix B, Work Package Guidelines. NNFD required reviewers/approvers are indicated in the Work Package Reviewers Table section of NNFD-FRM-058, Work Package Forms and NNFD Post Maintenance Testing direction is provided in Appendix C, Post Maintenance Testing.

Assemble and Allocate Resources –Resources are allocated on a priority basis for NNFD based on safety, research priorities, and other factors. Craft resources are allotted by the Work Management and Maintenance group. This work prioritization is communicated to all involved personnel via the online scheduling system.

Hold Points –Hold points ensure the necessary safe work environment and prerequisite conditions for the workers to safely and effectively perform the requested work. Use of hold points should be limited to those steps, whose omission would result in injury to personnel, damage to equipment, or serious non-compliance to a requirement. Hold points require the signature or initials of the individual responsible for verifying the hold point requirements, and continuation in the work plan shall not occur until the required signatures are obtained.

Work Log – The Work Log Section of NNFD-FRM-058, Work Package Forms is a chronological log of significant events or findings such as work package review comments, revisions and approval, scope change, status of work for complicated packages, parts used, feedback comments, and changes that were made such that a person not involved with the task can understand what was done by reading the work package. For Standing Work Packages, it may also include daily start authorization, pre-job brief documentation, hold point approvals, and closeout authorization. A Work Log is required for inclusion in all work plans. The Work Log section of NNFD-FRM-058, Work Package Forms may be formatted to best meet work package requirements.

Conduct Work – This is the actual physical execution of the work to resolve the originator’s request. The assigned Task Leader has specific responsibility and authority to accomplish the work. The Task Leader coordinates all tasks and assumes responsibility for ensuring the tasks are performed accurately, safely and in compliance with the specified administrative and technical requirements. Pre-job briefs are conducted per NNFD-018, Pre-Job Briefs and Post-Job Reviews.

Additionally, the Task Leader ensures that post-maintenance testing (Appendix C) has been completed to ensure the proper functionality or operability of the system or component, as appropriate.

Closure and Feedback - This ensures the quality of all work meets NNFD and customer requirements and that a continuing improvement process is in place. Task Leaders are responsible and accountable for the functional and operational concerns associated with the work performed. Feedback is performed in accordance with NNFD-018, Pre-Job Briefs and Post-Job Reviews.

Closure reviews (Work Package Closure Checklist section of NNFD-FRM-058, Work Package Forms) are necessary for all completed work packages prior to archiving. The Task Leader is responsible for this closure review to ensure the completed work packages contain all the necessary items important to operations, safety, quality, engineering, configuration control, environmental protection and waste management concerns prior to final records retention. This may include the development and issuance of new or revised drawings, procedures, preventive maintenance, spare parts, and training.

4. PROCESS

4.1 Work Identification

ALL PERSONNEL

NOTE 1: If conditions present a personnel safety risk, a TSR issue, OR may be ORPS reportable, the Facility Management Group Lead or designee shall be notified immediately.

NOTE 2: For operations activities which will utilize an Operations Work Plan, Work Order creation and screening is not required.

4.1.1 If a request for work is needed, submit a service request electronically using SBMS Subject Area, Electronic Service Request Process.

Anyone may identify a deficient condition or request needed work through NNFD by submitting a service request. A service request is simply a request for work that may come in different forms. Service requests might be a verbal request to the service provider, an electronic request through the electronic service request system or programmed maintenance request (QE&I inspections, calibrations, preventive maintenance and recalls) via the Computerized Maintenance Management System (CMMS). Service requests serve to identify the work.

NNFD FMGL/TL

4.1.2 If a request for work is submitted verbally, ensure that the request is entered into the electronic service request system.

4.2 Work Screening

NNFD FMGL/TL

4.2.1 Screen the work request for personnel safety, TSR issues, and immediate mission impact.

4.2.2 Screen the service request via the CMMS for grade, priority and status.

Screening (Graded Approach)

For NNFD the work screening is the process by which the NNFD Facility Management Group Lead or his/her designee performs the work screener function for a service request. The work screener considers funding, safety, and competing priorities prior to approving a work request. The purpose of this activity is to understand the specific work identified/requested. The screening function is comprised of categorizing the work by grade, (Grade 1 (A-H), Grade 2, Grade 3, Grade 4) by priority (1-8) by status (On hold for Additional information, Approved for Planning, or Deferred) or canceling the request. The work screener accomplishes this task by entering this information through the CMMS for the Work Order.

Work Control Page No. 10 of 36

DEFINITIONS

1. Approved for Planning - Job is ready to be planned. A Job is normally placed in this status by the NNFD Facility Management Group Lead when the job is ready to be sent to the task leader for planning.

2. Cancel – the work order is rejected. If the request for service is rejected for any reason, then the Work Screener shall notify the requester of the reason for canceling.

3. Deferred - Work items or facility deficient conditions that are deferred (due to: lack of funding, life cycle, etc.). Gives representation of true facility condition and provides a method to track and review.

4. Emergency Work – Work required to mitigate an imminent or existing nuclear or personnel safety hazard.

5. GRADE 1 - Work that could affect the operation of a CAT 1, 2, or 3 Nuclear Facility. Grade 1 tasks are subdivided into 8 types (A-H) based on the definitions in the table below. These types are driven by the documentation and review requirements for work being performed. All work in NNFD CAT 1, 2, OR 3 nuclear facilities are considered Grade 1.

Work Type Description

A May not effect operation of facility, however facility management determines that work Requires Facility

Management Group Lead Attention

B Experiment Systems Maintenance

C Non-Safety Related (SC/SS/DID)* Preventive Maintenance (PM), Calibrations, or Routine Operations

D Non-Safety Related (SC/SS/DID) Corrective Maintenance (CM), Trouble Shooting/Testing, or Routine

Operations

E Safety Related (SC/SS/DID) Preventive Maintenance (PM), Calibrations, or Routine Operations

F Safety Related (SC/SS/DID) Corrective Maintenance (CM), Trouble Shooting/Testing or Routine Operations

Work Control Page No. 25 of 36

6. GRADE 2 - Work that could not affect the operation of a CAT 1, 2, or 3 Nuclear Facility. (Life Safety) Non-nuclear functional features relative to fire, storm, collapse, crowd behavior, and other related safety considerations within a structure or facility. (High Rigor Equipment) Equipment for which maintenance or repair requires a higher degree of rigor in work planning, performance and documentation, as specified in regulations or standards due to potential for serious injury. This systems/equipment may have defined work protocols required by OSHA, a nationally recognized standard such as ANSI or ASME, or by the equipment manufacturer. Examples: cranes, fork lifts, man lifts or work platforms, pressure vessels, high pressure systems, etc. (Facility Modifications) Significant changes to a facility that are governed by Building, Fire, or other recognized codes or standards. NOTE: Additional guidance is provided in the procedure “Implement Work Control for Operations, Maintenance, and Services” (see Work Grading Guidance).

7. GRADE 3 - Work that could not affect the operation of a CAT 1, 2, or 3 Nuclear Facility. Work activities not classified as Grade 1 or 2 that require job specific permitting for personnel safety or a high level of coordination due to the fact that the work might have a significant impact on laboratory operations. NOTE:

Additional guidance is provided in the procedure “Implement Work Control for Operations, Maintenance, and Services” (see Work Grading Guidance).

8. GRADE 4 - Work that could not affect the operation of a CAT 1, 2, or 3 Nuclear Facility. Work activities that can be completed and controlled effectively through skill-of-the-craft. NOTE: Additional guidance is provided in the procedure “Implement Work Control for Operations, Maintenance, and Services” (see Work Grading Guidance).

9. High Impact – Work that involves lifting of, or maintenance on (with risk of permanent damage) equipment with replacement costs and labor valued at or over 10 million dollars, emergency work, non-routine transportation of quantities of nuclear material > Category 3.

10. Hold Point – A Hold Point is a step in the Work Plan that identifies the necessary safe work environment and prerequisite conditions for the workers to safely and effectively perform the requested work. A Hold Point requires facility conditions to be verified or changed and require a signature from the cognizant discipline (i.e. FMGL, RCT, IH, Quality) prior to proceeding in the work plan.

Hold points are placed at the discretion of the work plan author and used to identify critical steps, changes in operating or safety conditions, or other instances where proceeding beyond the hold point without having accomplished the requirements of the hold point would produce dire results.

Hold points require the signature or initials of the individual responsible for verifying the hold point requirements, and continuation in the work plan shall not occur until this has been accomplished.

• Example – A work plan requires a specific system alignment for work to continue. This would require a Hold Point which would only be released by the Facility Management Group Lead’s signature, once the system alignment is complete.

• Example – A work plan requires intrusion into a potentially contaminated system. This would require a Hold Point which would only be released by a Radiological Control Technician’s signature, once the work area has been surveyed to meet the specifications in the work package.

11. Materials – tools, equipment, parts, etc. staged for job.

12. On Hold for Additional Information – This status is assigned to a service request that does not provide the initial information needed to correctly grade and disposition the work.

13. Ready to Work - Paper work complete – Job Hazard Evaluation, Permits, Work Instructions, reviews and approvals.

14. SMWP - Standing Maintenance Work Package – A work package that is used to perform corrective, preventive, or predictive maintenance and is intended to cover more than one occurrence of the work scope.

Work Control Page No. 26 of 36

15. Urgent Work – Work necessary to maintain continued operation of, or address an identified deficiency that may result in a nuclear or personnel safety hazard, or other abnormal condition in a nuclear facility that cannot be addressed in a timely manner through the normal work control process without undue risk, hazard exposure, or consequence.

Priorities

1 Correction or mitigation of imminent safety hazards: This category includes industrial safety, radiological safety, environmental protection, security (physical, nuclear material) situations where direct action is needed to correct, mitigate, prevent risk to the public, workers, or the environment.

2 Corrective and preventive maintenance of nuclear safety-class and safety significant configuration items and comparable systems/items for other hazardous materials: This category includes maintenance of systems and components that are necessary to…
