About this file
This attachment is a Logic Model Guidance document provided by Pinellas County, Florida, for use in developing proposals for indigent burial and cremation services. The document serves as a template to help qualified providers structure their program submissions by outlining key components including project goals, inputs, activities, target populations, monthly metrics, and expected outcomes. Providers are instructed to describe the broad, long-term changes their program intends to achieve and to identify the resources they will dedicate to service delivery, including staffing, technology, training, and funding. The guidance requires applicants to specify core programmatic actions, frequency and method of service delivery, the number and characteristics of clients to be served, and eligibility criteria or service priorities, with anticipated reach projected monthly, quarterly, or annually.
The document emphasizes the development of quantifiable and measurable performance indicators that are reportable monthly and directly tied to program activities and objectives. Applicants must identify a minimum of three outcomes demonstrating specific, measurable changes in clients or the community resulting from program activities, including metrics such as percent change or number of participants impacted. All metrics must be realistic, aligned with program objectives, and focused on results rather than activities or process steps. This Logic Model framework enables Pinellas County to evaluate how qualified burial and cremation service providers will invest resources, conduct operations, serve eligible populations, track progress, and achieve meaningful community outcomes.
