About this file

This is a questionnaire attachment (Appendix C) for a state and local contract opportunity issued by the South Carolina Department of Juvenile Justice (SCDJJ) for Teen After-School Centers (TASC). The document serves as a bidding requirement that prospective centers must complete and return with their proposals. Bidders must provide detailed responses explaining how their centers will meet ten specific program requirements and responsibilities for serving at-risk youth and juveniles under probation supervision for nonviolent offenses aged 12-17. The questionnaire addresses the center's programmatic approach, summer enrichment planning, staff screening and training protocols, facility specifications, supply provision, mentor availability, kitchen access for snacks, recreational equipment and space, monthly reporting procedures to SCDJJ, and compliance with daily activity requirements including snacks, recreation, academic enrichment, service projects, and life skills development.

The questionnaire requires bidders to confirm facility specifications including at least one classroom accommodating fifteen youth and two adults, access to indoor recreation areas, and kitchen facilities or access for snack preparation. Staff and volunteer requirements mandate screening through SCDJJ, approval by the National Crime Information Center, and South Carolina Department of Social Services clearance, with mandatory attendance at a one-day SCDJJ training on working with at-risk youth. Centers must maintain at least two adults in the classroom and seek to provide mentorship to each youth participant throughout program duration. The document requires detailed descriptions of available equipment, recreational activities, supplies, and monthly written reporting on financial and programmatic outcomes, though specific contract pricing, award dates, and term information are not detailed in this questionnaire attachment.
