About this file
This document is an affidavit attachment for the Commonwealth of Kentucky's Security and Surveillance RFP Rebid (RFP 758 2700000043), establishing mandatory compliance requirements for bidders, offerors, and contractors seeking contract awards or renewals. The affidavits serve as attestations of legal and financial compliance and remain effective for one year from the date of execution. All bidding entities must submit a notarized Required Affidavit affirming that they have not knowingly violated Kentucky campaign finance laws and that contract award will not violate any campaign finance provisions under KRS 45A.110 and 45A.115.
The document outlines three separate affidavit options that contractors may claim based on their business status. Contractors may attest to general compliance with campaign finance laws, or they may claim resident bidder status by affirming authorization to conduct business in Kentucky, filing corporate income taxes for one year prior to bid advertisement, making payments to the Kentucky unemployment insurance fund, and maintaining an active workers' compensation policy. Alternatively, contractors may claim qualified bidder status as a nonprofit corporation furthering purposes under KRS Chapter 163 or as a qualified nonprofit agency for individuals with severe disabilities, which requires seventy-five percent of direct labor hours be performed by individuals with severe disabilities and maintenance of good standing with the Secretary of State. The bidding agency reserves the right to request supporting documentation for any claimed status, with failure to provide documentation resulting in bidder disqualification or contract termination.
