About this file

This document is an affidavit attachment for the Commonwealth of Kentucky's Fleet Car Wash Card Services Program RFP (RFP 758 2700000040). The attachment contains three required affidavits that bidders, offerors, and contractors must complete and submit to be considered for contract award or renewal. Each affidavit must be executed by an authorized representative of the contracting party, notarized, and returned to the Commonwealth. The affidavits are effective for one year from the date of execution.

The first affidavit requires contractors to attest under penalty of perjury that they have not knowingly violated Kentucky campaign finance laws and that contract award will not violate any provision of Kentucky campaign finance law. The second affidavit pertains to resident bidder status and requires bidders to confirm they are authorized to transact business in Kentucky, have filed Kentucky corporate income taxes for one year prior to contract advertisement, have made payments to the Kentucky unemployment insurance fund, and maintained a Kentucky workers' compensation policy in effect. The third affidavit addresses qualified bidder status under 200 KAR 5:410(3) and applies to nonprofit corporations furthering the purposes of KRS Chapter 163 or qualified nonprofit agencies for individuals with severe disabilities that employ such individuals for at least seventy-five percent of direct labor man hours. The bidding agency reserves the right to request supporting documentation for any claimed status and may disqualify bidders or terminate contracts for failure to provide such documentation upon request.
