About this file

This file is an affidavit document for the Commonwealth of Kentucky related to the Kentucky 811 Training Module contract opportunity (RFP 758 2600000115). The document serves as a required submission for bidders, offerors, and contractors seeking to be awarded or renew a contract with the state. The affidavit is effective for one year from the date of execution and must be completed by an authorized representative of the contracting party, notarized, and returned to the Commonwealth before contract award.

The document contains three separate affidavit requirements that bidders must address. The first affidavit requires contractors to attest under penalty of perjury that they have not knowingly violated Kentucky's campaign finance laws and that contract award will not violate any campaign finance provisions. The second affidavit applies to bidders claiming resident bidder status and requires attestation that the entity is authorized to transact business in Kentucky and has, for one year prior to contract advertisement, filed corporate income taxes, made unemployment insurance fund payments, and maintained a workers' compensation policy. The third affidavit addresses qualified bidder status for nonprofit organizations, requiring documentation that the entity meets specific criteria under Kentucky statutes, including nonprofit corporation status furthering purposes under KRS Chapter 163 or qualification as a nonprofit agency for individuals with severe disabilities employing such individuals for at least seventy-five percent of direct labor hours. The bidding agency reserves the right to request supporting documentation for any status claims and may disqualify bidders or terminate contracts for failure to provide required documentation upon request.
