About this file

This is a Required Affidavit document for bidders, offerors, and contractors submitting bids to the Commonwealth of Kentucky. The affidavit serves as a compliance certification tool for the Kentucky 811 Training Module Re-Bid contract opportunity. Bidders and contractors are required to submit a notarized affidavit affirming compliance with Kentucky's campaign finance laws (KRS 45A.110 and 45A.115) prior to contract award or renewal. The affidavit remains effective for one year from the date of execution and must be completed by an authorized representative of the contracting party and notarized before submission to the Commonwealth. The document provides instructions for proper completion, including signature, printed name, title, date, bidder or offeror name and address, vendor code, notary information, and notary commission expiration date.

The affidavit document includes three distinct attestation sections that bidders may need to complete depending on their status. The first section requires contractors to swear under penalty of perjury that they have not knowingly violated Kentucky campaign finance laws and that contract award will not violate such laws. The second section pertains to resident bidder status claims, requiring bidders to affirm they are authorized to transact business in Kentucky and have maintained corporate income tax filings, unemployment insurance fund payments, and workers' compensation policies for at least one year prior to contract advertisement. The third section addresses qualified bidder status for nonprofit organizations serving individuals with severe disabilities, requiring verification of nonprofit registration and compliance with employment thresholds for disabled individuals. The bidding agency reserves the right to request documentation supporting any claimed status and may disqualify bidders or terminate contracts for failure to provide such documentation upon request.
