About this file
This document is an Insurance Schedule attachment for standard service contracts with the State of Nevada. The attachment outlines comprehensive insurance requirements that contractors and subcontractors must procure and maintain throughout the contract term, including any warranty periods. Contractors must obtain Commercial General Liability coverage with a general aggregate limit of $2,000,000 and per-occurrence limit of $1,000,000; Automobile Liability coverage with a combined single limit of $1,000,000 for owned, hired, and non-owned vehicles; and Workers' Compensation coverage at statutory limits with Employers' Liability limits of $100,000 per accident and $500,000 policy limit for disease. All policies must name the State of Nevada as an additional insured, with coverage extending to the full limits purchased by the contractor. Insurance must be placed with carriers licensed in Nevada and rated A- VII or better by A.M. Best.
Contractors must provide Certificates of Insurance (ACORD form or equivalent) before work commences and maintain all required policies for the duration of the project. Notice of cancellation requires thirty days' prior written notice to the State, except for non-payment of premiums which requires ten days' notice. Failure to maintain required insurance or provide evidence of renewal constitutes material breach of contract. The contractor's insurance must be primary and non-contributory to all other available sources. For subcontractors, the main contractor may either include them as additional insureds under its policies or provide separate certificates and endorsements. The State reserves the right to request complete certified copies of all insurance policies and may modify insurance requirements through administrative action by the Attorney General's Office or Risk Manager without formal contract amendment.
