About this file
This document is Attachment B, a Proposer Information and Certifications form for an Oregon state procurement process. The form requires proposers to complete key information and provide certifications affirming their compliance with various legal and ethical standards. The certifications cover understanding of RFP requirements, tax compliance, non-discrimination practices, subcontractor fairness, exclusion from restricted lists, proposal independence, and acknowledgment of potential legal consequences for false claims.
The form mandates proposers certify they have no undisclosed delinquent debt to the state, will not discriminate in employment or subcontractor selection, are not on federal exclusion lists, and have prepared their proposal independently. Proposers must also confirm compliance with civil rights statutes including the Americans with Disabilities Act, Title VI of the Civil Rights Act, and Oregon-specific regulations. The document serves as a legal declaration of the proposer's integrity, business practices, and commitment to meeting the state's procurement standards.
