About this file

This document is a Proposers Certifications and Representations form for RFP 10.6.25.1 issued by the David Douglas School District for Office Paper. The request seeks proposals for office paper to be delivered by the ream, carton, or skid, with the district reserving the right to award contracts to one or more qualified firms. The proposer must honor their pricing for 90 days from the date of signature and agrees to provide all necessary materials, services, and labor to perform work in strict accordance with the RFP requirements.

The document outlines several key certifications and representations, including non-discrimination commitments, independent contractor status, and verification of business practices. Notably, there are no specific pricing details in this attachment, but the proposer must complete the form, indicating their business designation (such as individual, partnership, corporation, etc.), and provide contact and identification information. The RFP includes provisions for proposal security, performance bonds, and payment bonds, though in this specific case, these are marked as "Not Required". The document also includes a section for potential certification by the Oregon Office of Minority, Women & Emerging Small Business (OMWESB) and requires bidders to declare their resident bidder status.
