About this file
This document is an Attachment B - Cost Response Template for a state and local contract opportunity issued by the Executive Office of Health and Human Services (EOHHS) in Massachusetts. The contract is to provide an Operations Manager to support the administration of the Wheelchair Loaner Program and Community Provider Wheelchair Repair Program. The template requires the bidder to provide cost estimates for startup costs, wheelchair loaner program, and wheelchair repair program over a 3-year period. The cost estimates are based on projected service volumes such as consumer phone calls, wheelchair repairs, and wheelchair loaners. Bidders must complete the provided cost tables and can include narrative to describe the assumptions used in developing the cost response.
The cost response template provides detailed instructions for completing the cost tables, including the required FTE counts and associated labor/fringe costs for various functional areas. Bidders must also account for other costs such as warehousing, inventory maintenance, software, and training. The template allows for the calculation of administrative overhead costs as a percentage of the total costs. The contract opportunity is for a single qualified entity to serve as the Operations Manager for these wheelchair programs.

Attachment B - Cost Response Worksheet
Instructions The Bidder must submit a cost response in accordance with the instructions presented in this Attachment B. EOHHS requires that the attached cost tables be completed as they are presented. Failure to complete the tables may disqualify the Bidder’s proposal. All assumptions or constraints made in arriving at the prices proposed in these tables must be clearly described in the Bidder's Attachment B Cost Proposal Narrative. Bidders are encouraged to include narrative to describe the assumptions on which this cost response is based.
Wheelchair Cost Proposal Instructions The Bidder must complete Section B of the cost response using the estimated service volumes in red in Section A, which are based on 12-month period. Bidders should provide costs in the green boxes of Section B using the instructions in Column B for each year, broken out by Start-up Costs and each of the Wheelchair Programs. All other boxes will auto-calculate based on the Bidder's responses.
