About this file

This document is the Cost Response Template for a Request for Response (RFR) issued by the Executive Office of Health and Human Services (EOHHS) of the Commonwealth of Massachusetts. The RFR solicits proposals from qualified bidders to serve as an Operations Manager to support the administration of the Wheelchair Loaner Program and Community Provider Wheelchair Repair Program. The required services include fielding consumer phone calls, dispatching for wheelchair repairs and loaner services, delivering and picking up loaner wheelchairs, and performing wheelchair repairs. The RFR specifies estimated service volumes over a 12-month period, including 10,713 consumer phone calls, 9,889 wheelchair repairs, and 1,000 wheelchair loaners.

The Cost Response Template provides detailed instructions for bidders to complete the required cost proposal, including startup costs, annual costs for the Wheelchair Loaner Program and Wheelchair Repair Program, and administrative overhead costs. Bidders are required to provide the number of full-time equivalent (FTE) staff dedicated to each functional area and the associated labor and fringe benefit costs. Other required cost elements include warehousing, inventory maintenance, software, and training. The RFR specifies a 3-year contract term with the possibility of renewal.

BIDDER NAME:

Attachment B - Cost Response Template (Amended and Restated February XX, 2023)

Instructions The Bidder must submit a cost response in accordance with the instructions presented in this Attachment B. EOHHS requires that the attached cost tables be completed as they are presented. Failure to complete the tables may disqualify the Bidder’s proposal. All assumptions or constraints made in arriving at the prices proposed in these tables must be clearly described in the Bidder's Attachment B Cost Proposal Narrative. Bidders are encouraged to include narrative to describe the assumptions on which this cost response is based.

Wheelchair Cost Proposal Instructions The Bidder must complete Section B of the cost response using the estimated service volumes in red in Section A, which are based on 12-month period. Bidders should provide costs in the green boxes of Section B using the instructions in Column B for each year, broken out by Start-up Costs and each of the Wheelchair Programs. All other boxes will auto-calculate based on the Bidder's responses.
