About this file

This document is a Cost Proposal Attachment (Amendment #1) for a Request for Proposal (RFP #11142024MC) issued by the University of Utah Health for trash can liners. The RFP seeks a qualified vendor to supply 17 different types of can liners, including white, clear, bio waste, healthcare, and recycling liners, with varying specifications such as thickness, size, and quantity per case. The proposal covers a potential three-year contract term, with projected monthly usage of 12 months for each line item. Vendors are required to complete the cost proposal form by entering numeric values in yellow highlighted fields, with strict instructions that any deviation could render the proposal non-responsive.

The pricing structure for this contract will be competitively scored, with the lowest proposal receiving 100% of price points and other proposals receiving proportional points based on their price relative to the lowest bid. Proposals more than double the lowest price will receive zero points. The document includes a detailed matrix for vendors to quote case costs for each liner type, with projected usage quantities that will be used for scoring calculations. The total contract value is not explicitly stated, but vendors must provide precise pricing for each of the 17 different can liner specifications, covering potential usage across three contract years.

Inclusion of any cost or pricing data within the proposal requirements may result in your proposal being judged as non-resposive. Any deviation from this cost proposal attachment "B" may deem your proposal non-responsive.

Cost Proposals will be scored as follows: The points assigned to each offerors cost proposal will be based on the lowest proposal price. The offeror with the lowest Proposed Price will receive 100% of the price points. All other offerors will receive a portion of the total cost points based on what percentage higher their Proposed Price is than the Lowest Proposed Price. An offeror whose Proposed Price is more than double (200%) the Lowest Proposed Price will receive no points. The formula to compute the points is: Cost Points x (2- (Proposed Price/Lowest Proposed Price)).
