About this file
This document is Attachment A - Minimum Mandatory Requirements for a Request for Proposal (RFP #11142024MC) issued by University of Utah Health (UUHC) for trash can liners. The RFP outlines seven critical vendor requirements that will be evaluated on a pass/fail basis, with vendors required to indicate their capability to meet each requirement by marking "YES" or "NO" in designated cells. Key mandatory requirements include the ability to deliver products five days per week, provide substitute items within 48 hours when original items are out of stock, conduct semi-annual site visits to review customer satisfaction, maintain added safety stock for start-up locations, provide a dedicated account representative who responds to emails within 24 hours, submit requested samples by the specified due date, and guarantee replacements for late deliveries.
Vendors who fail to respond to any of the seven requirements or answer "NO" to any requirement will be deemed non-responsive and will not proceed to subsequent evaluation stages of the RFP. The document emphasizes strict compliance with these minimum mandatory requirements as a prerequisite for consideration in the procurement process. No specific pricing information, funding sources, or contract term details are provided in this attachment, which appears to be a preliminary screening document focused on establishing vendor capability and commitment to meeting UUHC's operational needs for trash can liners.
