About this file
This is a Closeout Specifications attachment document for the Illinois Department of Innovation and Technology (DoIT) related to contract B-50657, a rebid for Fiber Construction Services in the North Region. The document outlines the vendor's work acceptance process, which requires the vendor to submit a substantial completion notice, incorporate any incomplete work corrections within an established timeframe, and submit redlined as-built drawings in Adobe PDF format along with GIS ESRI Shape files and fiber test results in compliance with Data Submittal guidelines detailed in Attachments C and D. The Department will conduct a punch list walk-through to identify deficiencies, and the vendor must complete all corrections within a specified timeframe and submit them for DoIT clearance via email.
The vendor must provide supplemental documentation including warranties, guarantees, maintenance data, instruction manuals, a detailed list of extra materials and equipment owned by the State, and a final Waiver of Lien from all subcontractors and suppliers. Following correction of punch list deficiencies and receipt of all required documentation, DoIT or its authorized engineer will conduct a final inspection. The project is considered complete and accepted only when DoIT confirms that all build work requirements and closeout documentation have been satisfactorily completed.
