About this file
This document contains two appendices providing procedural guidance for a federal contract opportunity involving construction or renovation work at a Department of Veterans Affairs (VA) facility in Southern Nevada.
Appendix A outlines the VA Personal Identity Verification (PIV) Card Access Procedures that contractors and personnel must follow to gain facility access. The process includes four main stages: submission of forms (OF 306 and Self-Certification of Continuous Service), PIV sponsorship by VA, fingerprinting through the USAccess Assured Identity Scheduler at the VA Southern Nevada Healthcare System location in North Las Vegas (6900 N Pecos Rd, North Las Vegas, NV 89086-4400), and badge receipt and activation. Personnel must provide acceptable forms of identification (primary and secondary) and complete a three-calendar-day eApp questionnaire through Okta or National Background Investigation Services (NBIS). Upon badge issuance, personnel must verify access at the security desk and scan QR codes at facility entrances for check-in and check-out during construction activities.
Appendix B provides the VHA Infection Control Risk Assessment (ICRA) template for construction, renovation, and maintenance activities at VA healthcare facilities. The form requires contractors to categorize work activities (Categories A through D, ranging from inspection and facility upkeep to large-scale construction), identify affected areas and patient populations, determine overall patient risk levels (Low, Medium, High, or Highest Risk), and implement corresponding infection prevention and control precautions (Levels I through IV). The template includes detailed control measures for each precaution level, including dust containment, air handling, barrier construction to NFPA 241 standards, HVAC system management, water system flushing, and post-completion cleaning and verification. An alternative interior exhaust procedure is provided as an option to outdoor exhaust requirements for facilities unable to exhaust to the building exterior.
