About this file
The document is a Mandatory Features Checklist for a group dental insurance Request for Proposal (RFP) issued by the Brevard County Board of County Commissioners, targeting a dental insurance contract effective January 1, 2026. The RFP seeks a fully insured dental insurance provider for active employees, retirees, and eligible dependents, with a proposed five-year contract term. Proposers must submit three hard copies and one electronic copy of their proposal, with all mandatory features checklist items completed. Key requirements include being licensed in Florida for at least five years, having experience insuring groups of 4,000+ employees, offering no minimum participation requirements, and providing dental DHMO and PPO plan options.
The checklist outlines specific mandatory qualifications, including permitting retired employees and dependents to continue coverage, matching the county's dependent definition (up to age 30), and serving as the prime contractor performing at least 80% of contract work. The proposer must maintain a single contract with separate billing for each agency and provide benefits that meet current coverage limits. While no specific budget is detailed in this document, the related contract opportunity summary indicates historical annual premiums around $2.1 million, suggesting a potential five-year contract value of approximately $10.5 million. The incumbent vendor is Cigna, with current monthly rates ranging from $12.91 to $106.07 depending on plan and coverage tier, and approximately 4,210 total enrolled participants.
