About this file

This is a sample contract template from the Washington State Department of Children, Youth & Families (DCYF) for Family Resource Center (FRC) Referral and Support Services. The contract establishes the terms and conditions under which a contractor will provide voluntary, community-based prevention and support services to families exiting child welfare involvement. Services include family needs assessment, service navigation, referrals to community-based supports, and connections to resources addressing housing, food, childcare, public benefits, and other basic needs for families with children over age three in high-need communities. The contract period runs from January 1, 2027, through June 30, 2027, with an initial six-month ramp-up period during which the contractor will onboard staff, complete required trainings, and establish partnerships with DCYF regional offices and public health services. All FRC staff must complete mandatory DCYF trainings including mandated reporter training, FRC onboarding, trauma-informed care, cultural responsiveness, and data management system training. The contractor must contact referred families within three business days, complete intake and family strengths assessments within ten business days, and close service navigation within 60 days of initial referral, with performance targets requiring 100% documentation of assessments and service navigation plans.

The maximum contract amount is $37,600.00 for the fiscal year ending June 30, 2027, with compensation structured through three billing mechanisms: capacity payments per full-time Family Advocate position per month, referral rates per accepted family referral, and case rates for delivering all specified services once enrollment begins. During the six-month ramp-up period, the contractor receives enhanced capacity payments to support heightened program promotion and initial service delivery; after ramp-up, standard capacity, referral, and case rates apply according to DCYF's fee table. The contractor must maintain comprehensive insurance including general liability ($1,000,000 per occurrence/$2,000,000 aggregate), business automobile liability ($1,000,000 combined single limit), and professional liability ($1,000,000 per occurrence/$2,000,000 aggregate), with DCYF named as additional insured. The contractor must comply with strict data security requirements including encryption standards, access controls, secure storage, and breach notification within one business day to dcyf.contractdatabreach@dcyf.wa.gov. Invoices are submitted monthly on Form A-19 to the designated DCYF address, with payment due within 30 days of receipt of properly completed invoices and withholding possible for delinquent or incomplete reports.
