The document is a Quality Assurance Surveillance Plan (QASP) for hazardous waste disposal services at the Kansas City VA Medical Center. The QASP outlines comprehensive surveillance methods to ensure contractor compliance with VA regulations, including direct observation, periodic inspections, random sampling, performance metrics review, and customer feedback. Surveillance will be conducted monthly for most performance areas such as waste removal procedures, documentation, safety protocols, and equipment maintenance, with quarterly reviews of contractor licensing and overall performance.
Key performance metrics include completion rate of hazardous waste pickups, documentation accuracy, safety protocol compliance, emergency response times, equipment maintenance, and contractor certification status. The plan defines roles for the Contracting Officer, Contracting Officer's Representative (COR), and Safety Officer, who will monitor and document the contractor's performance. The contract details are currently to be determined (TBD), including the contract number, COR, and period of performance.
