About this file

This is an Environmental Testing Protocol attachment outlining the Legionella Detection Program requirements for the George E. Wahlen VA Medical Center. The contractor is required to collect water samples for Legionella testing at distribution endpoints across multiple buildings including patient care areas (Buildings 1, 3, and 14), non-patient buildings (32, 42, and 43), and the cooling tower (Building 27). Testing frequency varies by month with core testing sites designated at Building 1 Cold Tank Room, Building 27 Cooling Tower, and three units within Building 42, plus the Building 43 Pumphouse. The contractor must collect samples using "first draw" methodology, filling specimen containers with approximately 125 ml from standard outlets and 1000 ml from hot and cold water sources, then shipping samples to a CDC ELITE-certified laboratory within 24 hours.

The protocol specifies that water samples must be analyzed at a CDC Environmental Legionella Isolation Techniques Evaluation certified laboratory, with positive results reported to the Contracting Officer's Representative within 24 hours of receipt to minimize exposure risks to patients. All results must be reported using the template format provided in Attachment E. New outlet sites are selected for each testing period, though any previously positive sites require retesting at the next testing cycle. Additionally, post-construction Legionella testing must be conducted per Infection Control Risk Assessment requirements, and recovery procedures including biocide flushing must be performed following any water line disturbances. The monthly testing plan details specific numbers of sites and tests for each building across three rotating monthly schedules totaling 34 tests per month across 14 sites.

ATTACHMENT A

GEORGE E. WAHLEN VA MEDICAL CENTER

ENVIRONMENTAL TESTING PROTOCOL (Legionella Detection Program)

1. The determination of the sampling sites is controlled by Engineering Service.

1. Water testing frequency for Legionella is controlled by VHA Directive 1061, the VASLCHCS, and with the advisement of the Infection Preventionist.

2. The contractor will collect water samples for Legionella testing at distribution endpoints in patient care areas of buildings 1, 3, and 14, and non-patient buildings 32, 42, and 43, and the cooling tower (building 27). The Sampling Plan below outlines monthly testing requirements for each quarter.

a. Core Testing Sites:

Building 1 – Cold Tank Room Building 27 – Cooling Tower Building 42 – Unit 1 Soft Building 42 – Unit 2 Hard Building 42 – Unit 3 Hard Building 43 – Pumphouse

5. New outlets will be chosen for each testing period. Any sites that were positive for Legionella during the previous testing will be re-tested the next time testing is conducted.

6. Water samples will be analyzed at a Centers for Disease Control and Prevention (CDC) Environmental Legionella Isolation Techniques Evaluation (ELITE) certified lab.

7. Contractor will collect the water samples and ship them within 24hrs to the testing lab. Water samples from outlets will be collected as follows:

a. Samples must be “first draw”.

b. Fill the specimen container provided by the testing laboratory with approximately 125 (ml) of water.

c. Label the container with the sample number.

d. Two water samples from hot only source:

(1) Open the drain valve and immediately fill one specimen container with 1000 ml of water. Label the container with the sample number.

e. Two water samples from cold only source:

(1) Open the drain valve and immediately fill one specimen container with 1000 ml of water. Label the container with the sample number.

8. The Contractor will provide positive sample results within 24hrs of receipt to minimize exposure risks to patients.

9. All results will be reported in the template format provided in Attachment E.

10. Post-Construction Legionella testing will be conducted per the requirements of the Infection Control Risk Assessment conducted by the Infection Control Preventionist for each applicable project.

11. Following any disturbance to the water line, recovery procedures will be performed to ensure biocide is present at the most distal site affected by the disturbance. This includes flushing the line until an adequate biocide concentration is reached.
