About this file
This is an Insurance Certification attachment to RFP 25SS0022 issued by the California Department of Veterans Affairs (CalVet) for Financial Governance consultation services. The document outlines the insurance requirements and certification obligations that proposers must acknowledge and comply with upon contract award. Proposers are required to review and understand the insurance requirements detailed in Attachment 15 (Sample Contract, Exhibit D) and to certify their ability to provide all required insurance documentation in a timely manner following contract award, including certificates of insurance and proof of coverage for the State of California, its officers, employees, and agents. The Insurance Certification form must be submitted with proposals, though actual proof of insurance documentation is only required from firms that are awarded contracts.
The document specifies that if Commercial General Liability insurance is required and subcontractors are used to complete portions of the agreement, proposers must either include subcontractors as additional named insureds under the proposer's policy or ensure that each subcontractor maintains their own Commercial General Liability policy. Proposers must provide proof of insurance coverage through either an endorsement to their insurance policy (Form CG 20 10 11 85 or equivalent) or a copy of their current insurance policy demonstrating coverage of all required parties. CalVet reserves the right to cancel a proposed award if the selected proposer fails to submit required insurance documentation. The form serves as an acknowledgment that proposers understand these requirements and certify their compliance capability.
