Standard Operating Procedure for DOH Water Supply Improvements
NYSDOH requires the following projects to be submitted for review:
Any Water line projects >1,000 ft
New production wells
Modifications to: water storage tanks, reservoirs, etc.
Potable water treatment facilities – new and upgrades
All Backflow Prevention Device Installations – use form DOH-347
Responsibilities:
Public Works Environmental Compliance Drinking Water Program Manager will manage all Army Corp of Engineers and Public Works Engineering Applications for Approval of Plans for Public Water Supply Improvement (DOH-348) application submittals to DOH.
Other projects such as Mountain Community Homes or Contractor Owned Construction the A/E Project Company and Project Manager will manage their own Drinking Water application submittals but keep Public Works Environmental Compliance Drinking Water Program fully informed by letter.

Army COE and PW Engineering Procedure:
1. COE or PW Engineering will review drinking water submittal to verify design meets standards, drawings have engineers stamp and signature and Fort Drum Cultural Resources consultation has been completed.
2. NYSDOH Application for Approval of Plans for Public Water Supply Improvement (DOH-348) must be filled out and supplied to Public Works – Environmental Compliance, Drinking Water Program Manager along with 3 copies of the design submittal.
3. Backflow Prevention Device Applications (DOH-347) are also required as part of the submittal package and will be copied to the Backflow Coordinator.
3. Water Program Manager will have the Public Works Director sign DOH Application for Public Water Supply Improvement and Backflow Prevention Device Applications.
4. A transmittal letter to DOH will be generated and signed by Public Works Environmental Chief. The entire package will be transported to DOH within 2 days.
The transmittal letter will be stamped received by DOH. The stamped cover letter will be scanned and emailed to Project Manager, and Backflow Coordinator.
5. DOH will respond to submittals by letter. Incomplete packages that require clarification or resubmittals the DOH response letter will be addressed to the Drinking Water Program Manager. Resubmittals will be routed through the Drinking Water Program Manager for tracking purposes. DOH Authorization to construct letters will be sent to Public Works Director. Drinking Water Program Manager will pick up DOH stamped drawings and distribute to Project Manager.
6. Certification of completed project by designer of record must be reviewed by COE or PW Engineering for accuracy and completeness then given to Drinking Water Program Manager. A/E will witness the testing and certify that project was constructed in accordance to the approved plans. Deviations from approved plans require as-builts along with the testing results. A transmittal letter to NYSDOH will be generated and signed by Public Works Environmental Chief. The certification package will be transported to DOH within 2 days. The transmittal letter will be stamped received by DOH. The stamped cover letter will be scanned and emailed to project manager.
7. DOH will review and accept certifications and respond to the Project Leader with cc to Public Works Director and Drinking Water Program Manager.

Fort Drum Mountain Community Homes or Contractor Owned Construction:
1. Cultural resources consultation must be begun as soon as a project is in the design phase. Submit site footprint (pdf, .dgn and shape file) and estimated scope of the water project to Cultural Resources and Drinking Water Program Manager.
2. Contractor Owned Construction will sign DOH Application for Approval of Plans for Public Water Supply Improvement (DOH-348) and Backflow Prevention Device Applications (DOH-347) and submit application and 3 copies of the design submittal to DOH. Copies of plans and letters to Drinking Water Program Manager.
3. DOH will respond to submittals by letter. Incomplete packages that require clarification or resubmittals will be handled by the A/E Project Company. DOH Authorization to construct letters will be sent to A/E Project Company. Copy of authorization to construct will be routed to Drinking Water Program Manger.
4. Certification of completed project by designer of record will be submitted by A/E Project Company to DOH. Copy of certification transmittal letter will be routed to Drinking Water Program Manager.
5. Copy of DOH acceptance of completed project will be forwarded to Drinking Water Program Manager to close out project.
Part 5, Subpart 5-1 Public Water Systems
5-1.22 Approval of plans and completed works. (a) No supplier of water shall make, install or construct, or allow to be made, installed or constructed, a public water system or any addition or deletion to or modification of a public water system until the plans and specifications have been submitted to and approved by the State. Materials used in the design, construction and repair of a public water system shall be lead-free. For this Subpart, lead- free shall mean solder or flux which contains no more than 0.2 percent lead and pipes, pipe fittings or any appurtenances which contain no more than eight percent lead.
(b) Approval of plans and specifications for public water systems shall be based on the following standards in their entirety. The State may allow deviations from these standards in accordance with procedures established by the commissioner.
(1) "Recommended Standards for Water Works", (Appendix 5-A)

(2) Standards for Water Wells (Appendix 5-B)
(3) Special Requirements for Wells Serving Public Water Systems (Appendix 5-D)
(c) The State may approve such plans or may require such modification which is deemed necessary to protect public health or safety. Application for plan approval shall be made on a form prescribed by the department.
(d) A supplier of water shall receive the approval of the State before placing into service any public water system constructed under the requirements of this section.
5-1.31 Cross connection control. (a) The supplier of water shall protect the public water system by containing potential contamination within the premises of the user in the following manner:
(1) by requiring an approved air gap, reduced pressure zone device, double check valve assembly or equivalent protective device consistent with the degree of hazard posed by any service connection;
(2) by requiring the users of such connections to submit plans for the installation of protective devices to the supplier of water and/or the State for approval;
