About this file
This document is an Asbestos-Containing Materials (ACM) Survey Report for the Salem District Office Building and Warehouse located in Salem, Oregon. The survey was conducted by Northern Industrial Hygiene, Inc. on January 8-9, 2014, with the purpose of identifying and assessing potential asbestos-containing materials throughout the facility. The investigation involved a comprehensive review of existing asbestos reports, visual inspections of interior and exterior building areas, and collection and laboratory analysis of 37 bulk samples from various materials including floor tiles, wallboard, paint, and thermal system insulation.
The laboratory analysis, performed by Northern Industrial Hygiene using Polarized Light Microscopy (PLM), revealed that only one of the sampled materials contained asbestos - a Brown 12" x 12" floor tile with mastic that contained 5% Chrysotile asbestos. The report provides detailed recommendations for contractors working in the building, including the need to be aware of potential concealed ACMs during maintenance or renovation activities, and advises that any future replacement materials should be inspected for asbestos prior to installation. The survey was conducted in compliance with OSHA, EPA, and other regulatory standards, with all samples analyzed by an NVLAP-accredited laboratory.
