About this file

This document is a Pre-Demolition/Renovation Asbestos Survey Report for the Ralph H. Johnson Veterans Affairs Medical Center in Charleston, South Carolina. The survey was conducted by Thunderbird Environmental, LLC in the building's crawl space during May and July 2023, with the purpose of identifying, quantifying, and sampling suspect asbestos-containing materials (ACM) that could potentially be disturbed during planned cleanup and repair activities.

Key findings include the discovery of asbestos-containing materials across four quadrants of the crawl space: approximately 12,000 linear feet of black mastic on fiberglass pipe insulation (30-40% significantly damaged), 13,000 linear feet of tan mastic on white covered fiberglass insulation (20-30% significantly damaged), and 500 linear feet of duct insulation with asbestos-containing black mastic (30-40% significantly damaged). The report recommends assuming all black and some tan mastic on fiberglass insulation contains asbestos, and suggests removing significantly damaged insulation in specific areas of each quadrant, particularly near stairs, exterior access points, and various column rows.

EXECUTIVE SUMMARY

Limited Pre-Demolition/Renovation Asbestos Survey Summary

Assessment Location: Ralph H. Johnson Veterans Affairs Medical Center located at 109 Bee Street, Charleston, South Carolina 29401, (referred to herein as “the Site”). Work was conducted in accessible areas throughout the crawl space under the facility. The crawl space was divided into 4 blocks – Block

A is the southeast quadrant, Block B is the northeast quadrant, Block C is the southwest quadrant and Block D is the northwest quadrant.

Purpose of Assessment: Perform a limited pre-demolition/renovation asbestos survey to identify, quantify, and sample suspect asbestos-containing materials (ACM), which could potentially be disturbed during the facility’s planned cleanup and repair activities throughout the crawl space under the facility.

Assessment Activities: The ACM survey was conducted on May 22-25 and July 10-15, 2023. Within the inspected area noted above, suspect materials were sampled to identify ACM with the potential for disturbance. Samples were submitted to CEI Eurofins (CEI) in Cary, North Carolina, a National Voluntary Laboratory Accreditation Program accredited laboratory (NVLAP code 101768-0), for analysis of asbestos content by the Polarized Light Microscopy (PLM) and where necessary Transmission Electron Microscopy (TEM) analysis methods. A summary of suspect materials included in the asbestos survey, with those confirmed to be ACM indicated in bold, is included in Table 1.

Key Findings

The following is provided as a summary of key findings from the asbestos survey:

All of the samples collected of black mastic on fiberglass insulation were positive for asbestos.

The estimated quantity of pipe insulation with asbestos-containing black mastic is 12,000 linear feet. Approximately 30-40% of the material is significantly damaged, 40-50% damaged, and 10-20% in good condition.

Two homogeneous areas of tan mastic on white covered fiberglass insulation were positive for asbestos and nine homogeneous areas of the similar tan mastic were negative for asbestos.

The estimated quantity of pipe insulation with tan mastic is 13,000 linear feet. 20-30% of the material is significantly damaged, 20-30% damaged, and 40-50% in good condition.

Samples from a previous report identified HVAC duct insulated with either one or two layers of fiberglass sealed by asbestos-containing black mastic. The estimated quantity of duct insulation with asbestos-containing black mastic is 500 linear feet. Approximately 30-40% of the material is significantly damaged, 20-30% damaged, and 20-30% in good condition.

iii

Key Findings

Reddish brown mastic on the duct joints under the insulation was also identified as asbestos containing. These were both identified in the SELC Report, May 2014 (HA-09, HA-10, HA-11).

Black mastic used to seal pipe and conduit penetrations through the slab above the crawl space should be assumed positive for asbestos.

Samples of Red firestop caulking on HVAC ductwork were all negative, except for one sample analyzed by TEM which indicated <1% Chrysotile.

All samples of the Calcium Silicate block pipe insulation collected in the crawl space were negative for asbestos. However, samples collected in a previous inspection identified the black mastic coating on the block insulation as positive for asbestos; SELC Report May 2014

(HA-04).

All samples of white mastic on white covered fiberglass insulation were negative for asbestos.

Tan and black mastic sampled on foam insulation were negative for asbestos.

One homogeneous area having mudded elbows on the pipe was negative for asbestos.

iv

Recommendations

Recommendations resulting from the limited pre-demolition/renovation asbestos survey include the following:

All black mastic on fiberglass insulation throughout the crawl space should be assumed positive for asbestos, in addition to all black mastic on block insulation.

All tan mastic on fiberglass insulation throughout the crawl space should be assumed positive for asbestos unless it has been tested and confirmed to be negative for asbestos.

All black mastic on the duct insulation throughout the crawl space should be assumed positive for asbestos.

All Black mastic used to seal pipe and conduit penetrations through the slab from above the crawl space should be assumed positive for asbestos.

Remove all significantly damaged insulation in the crawl space. The crawl space drawings are divided into four (4) “Blocks” or quadrants. Areas below are listed by Block, a list having significantly damaged asbestos TSI is as follows:

o Areas adjacent to all stairs and exterior points of access have damage from higher traffic areas. (Primarily black mastic and tan mastic on fiberglass pipe insulation)

1. INTRODUCTION

Thunderbird Environmental, LLC (Thunderbird) was contracted by Ralph H. Johnson Veterans Affairs

Medical Center (RHJ VAMC) located at 109 Bee Street Charleston, South Carolina (hereinafter referred to as “the Site”) to conduct a limited asbestos survey prior to planned cleanup activities throughout accessible areas of the building crawl space.

1.1. Purpose

The purpose of survey was to perform a limited pre-demolition/renovation asbestos survey to identify, quantify, and sample suspect asbestos containing material (ACM), which could potentially be disturbed during the cleanup activities and to provide representative data to assist in understanding what materials should be presumed to contain asbestos and what materials can be anticipated to not contain asbestos.

1.2. Scope of Services

This limited pre-demolition/renovation ACM survey was performed in accordance with South Carolina

Department of Health and Environmental Control (SCDHEC) and National Emission Standards for Hazardous Air Pollutants (NESHAP) regulations and included the following general components: (1) Site walkthrough, (2) Identification, classification, bulk sample collection of suspect ACM, and (3) interpretation of laboratory analytical results. Information provided by the Site contact about the specific areas and planned renovation tasks was used to determine the suspect material included in this survey. The scope of services provided by Thunderbird and HRP, during the completion of this limited pre-demolition/renovation ACM survey, included the following tasks:

Review of applicable regulatory standards on asbestos and pre-demolition/renovation requirements.

Coordination with a NVLAP accredited laboratory for bulk sample analysis.

Discussions of all areas of interest with the Site representative(s) and review of the scope of the pre-demolition/renovation project.

Identification, classification, quantification, and bulk sample collection of suspect ACM which could potentially be disturbed during the planned pre-demolition/renovation project; and

Interpretation of laboratory PLM and/or TEM analytical reports.

No significant scope-of-work additions or modifications other than those noted above were included in this limited pre-demolition/renovation ACM survey.

1.3. Project Limitations

This limited asbestos survey was focused solely on suspect materials with the potential for disturbance during the planned cleanup of the crawl space specified by the Site representative(s). This assessment may not have included all suspect materials, as the crawl space has inaccessible areas and only limited sampling of the suspect materials observed was included to provide representative assessment results to be followed throughout the crawl space. If any additional materials other than those sampled and reported in this survey are identified within these areas during the renovation activities, they should be treated as asbestos-containing until proven otherwise via sampling and analysis in accordance with SCDHEC regulations. It should also be noted that listed areas of damaged and significantly damaged materials and quantities are provided as approximations only, as detailed current as-built utility drawings were not available at the time of the survey and the inspectors had limited line of sight due to the restricted egress and complex utility configuration.

Specific areas that could not be safely accessed during the survey include the following:

Laundry/Engineering Addition

Chiller Plant

Area East of the Chiller Plant

2. ASBESTOS BULK SAMPLING

On July 10, 2023 to July 15, 2023, Mr. Frank J. Henry (BI-002037) of Thunderbird conducted a limited ACM survey at the Ralph H. Johnson VA Medical Center. In addition to Mr. Henry, additional survey support was provided by licensed inspectors from Trident Environmental Services, Inc., including Mr. Hunter

Hanson (BI-01468) and Kevin Leedy (ASB-20589), with onsite project management provided by HRP Associates, Inc. The scope of this survey was limited to the following specific areas and building components/systems as designated by Site contacts with the potential to be disturbed during planned cleanup and repair activities:

• Accessible portions of the crawl space throughout the building.

This survey was conducted in accordance with the requirements set forth by NESHAP for pre-renovation asbestos surveys outlined in the United States Environmental Protection Agency (US EPA) document 40

CFR Part 61 Subpart M, and as such, both destructive and non-destructive sampling techniques were employed by HRP to the extent necessary to collect all samples. This survey also complies with SCDHEC-specific requirements set forth in Regulation R.61-86.1 Standards for Performance of Asbestos Projects.

Reasonable effort was made during the survey to limit damage caused to the building and to eliminate the creation of a safety hazard or affect the overall integrity of the Site structures.

2.1. Previous Asbestos Survey(s)

One previous pre-demolition/renovation survey had been completed by Thunderbird and HRP for limited areas within the Site- specific areas of planned renovation. This included the following:

THU0007.BA – VAMC ACM Survey Report – Basement (August/September 2020)

In addition, historical sampling reports and supporting files completed by others were also provided by the site contact for reference. This included:

0359.E0001 – Ralph H Johnson VA Medical Center – Crawl Space – Limited Asbestos Inspection Report (SUMMIT Engineering, Laboratory & Testing, Inc., 3-21-2023)

Complete Crawlspace Steam Domestic HVAC Report (SELC, May 12, 2014) o The following positive asbestos identifications were utilized as a reference by the inspectors:

HA–03 Black Mastic on exterior of fiberglass pipe insulation;

HA-04 Black mastic on exterior of block insulation, calcium silicate type;

HA-05 White/Tan mastic on exterior of fiberglass insulation;

HA-09 Reddish-brown mastic on HVAC duct joints near stair 5; and

HA-10 & 11 Black mastic on fiberglass HVAC duct insulation near stair 5.

PipeBasement-Full-Layout1

Project 534-19-101 Design – Asbestos Testing (Durbin Environmental Consultants, Inc., April 22, 2020)

Ralph Johnson VAMC Charleston, SC – Asbestos Inspection Report – All Buildings – Dec. 2014 (USFIN Development, LLC, December 2014)

The above reports and supporting documents were utilized for reference only, and not for that of negative ACM confirmation based on historical sampling records. However, in certain instances as specifically noted above, previous positive ACM determinations were utilized as supplemental information to assist with identification and quantities and are indicated in Table 2.

2.2. Sampling Methodology

The inspectors systematically evaluated specific portions of the Site building which could potentially be impacted during future renovation activities, as described above, and specified by the Site representatives.

During the survey, the inspector classified each suspect ACM as one (1) of three (3) types:

Surfacing Materials - ACM sprayed or troweled on surfaces (walls, ceilings, structural members) for acoustical, decorative, or fireproofing purposes.

Thermal System Insulation (TSI) - insulation used to inhibit heat transfer or prevent condensation on pipes, boilers, tanks, ducts, and various other components of hot and cold-water systems and heating, ventilation, and air conditioning (HVAC) systems. This includes pipe lagging, pipe wrap;

block, batt, and blanket insulation; cements and “muds”; and a variety of other products such as gaskets and ropes; or, Miscellaneous Material – Other, largely non-friable products and materials such as floor tile, ceiling tile, roofing felt, concrete pipe, outdoor siding, and fabrics.

The inspector touched all suspect materials specified for disturbance by Site contacts to determine the extent to which the material, when dry, may be crumbled, pulverized, or reduced to powder by hand.

During the survey, suspect materials identified by the inspector were also evaluated for overall condition, and each suspect material was assigned a condition of significantly damaged, damaged, or good overall condition for the locations the materials were observed. As defined by the US EPA under Asbestos Hazard Emergency Response Act (AHERA), a material is significantly damaged if ten percent (10%) or greater of the material is evenly deteriorated, crumbling, hanging, or if the material contains over 25% localized damage. A damaged material is one that has less than 10% evenly distributed damage or less than 25% localized damage. A material in good condition is a material with no visible damage or deterioration or showing only very limited damage or deterioration.

Each suspect ACM was also classified into a homogeneous sampling area. An HA is an area which is uniform in texture and appearance, was likely installed during the same period, and is unlikely to consist of more than one type or formulation of material. The inspector collected samples of each material from each HA, with multiple samples of a single material collected as appropriate. Samples were not collected from materials that were readily identifiable as non-asbestos containing (i.e., wood, fiberglass, foam, rubber, paper wraps, etc.) or previously confirmed to be asbestos containing (if applicable).

To avoid disturbing the material any more than necessary and to minimize any unnecessary release of potential asbestos fibers during collection, the inspector performed bulk sampling of suspect ACM in accordance with generally accepted procedures outlined in the US EPA document "Guidance for

Controlling Asbestos-Containing Materials in Buildings" (EPA-560/5-85-024, June 1985). Each sample was collected using wet methods and placed in clean, sealable plastic bags and labeled with a unique sample identification number.

Bulk samples were submitted for analysis of asbestos content by Eurofins CEI Labs, Inc. (CEI), located in

Cary, North Carolina, a NVLAP accredited laboratory (NVLAP code 101768-0). CEI analyzed each material using polarized light microscopy (PLM) analysis method and where necessary using transmission electron microscopy (TEM) analysis method.

2.3. Non-Suspect Material(s)

Non-suspect materials, which are defined as materials that are not expected to contain asbestos, were not sampled, or analyzed. Non-suspect materials included black foam pipe insulation and general building materials (e.g., glass, metals, brick, fiberglass, concrete, wood, cinderblock, and rubber).

Please note that if a suspect material is later identified on-site and is not among those materials identified in this report, that material must be assumed to be asbestos containing until sampled and proven otherwise.

3. SITE PROVIDED INFORMATION

During the performance of this assessment, background information received from Site contacts, and/or previous assessment activities for this activity/task included the following:

3.1. Site Provided Information

The Site representatives specifically requested the inspection of TSI located throughout accessible portions of the crawl space. During this survey, Site contacts provided details on the planned cleanup and repair activities, in addition to providing the previously noted past asbestos survey reports as a reference.

4. FINDINGS AND CONCLUSIONS

As a result of this limited pre-demolition/renovation survey, the following is a list of findings and conclusions:

All of the samples collected of black mastic on fiberglass insulation were positive for asbestos, approximately 12,000 linear feet (LF). 3,600 LF is in block A, 3,300 LF is in block B, 3,700 LF is in block C and 1,400 LF is in block D. 30-40% of the material is significantly damaged, 40-50% damaged, and 10-20% in good condition. Included in the quantities - Thunderbird relied on asbestos sampling results from Complete Crawlspace Steam Domestic HVAC Report (SELC, May 12, 2014), Sample HA-03.

Two homogeneous areas of tan mastic on the white covered fiberglass insulation were positive for asbestos and nine homogeneous areas of the similar tan mastic were negative for asbestos. Of the estimated total approximate quantity (i.e., 13,000 LF), 3,500 LF is in block A, 3,700 LF is in block B, 3,200 LF is in block C, and 2,600 LF is in block D. 20-30% of the material is significantly damaged, 20-30% damaged, and 40-50% in good condition. Included in the quantities - Thunderbird relied on asbestos sampling results from Complete Crawlspace Steam Domestic HVAC Report (SELC, May 12, 2014), Sample HA-05.

Duct Insulation observed in the crawl space has asbestos-containing black mastic on the seams of the fiberglass insulation two layers and on the reddish brown mastic on the metal duct joints, including approximately 500 LF of 2’ x 2’ squared duct. Approximately 300 LF is on the west side of

Stair 5 and the other 200 LF is spread mostly equal throughout the rest of the crawl space. 30-40% of the material is significantly damaged, 20-30% damaged, and 20-30% in good condition. This is based on information provided in the SELC Report dated May 2014 (HA-09, 10 & 11).

Black mastic used to seal pipe and conduit penetrations through the slab above the crawl space should be assumed positive for asbestos. All samples of the Calcium Silicate block insulation collected in the crawl space were negative for asbestos. However, samples collected in a previous inspection identified the black mastic coating on the block insulation as positive for asbestos per SELC Report dated May 2014 (HA-04). Quantities of the black mastic coating on the block insulation are included above with the black mastic on fiberglass insulation.

All samples of white mastic on white covered fiberglass insulation were negative for asbestos.

Samples of red firestop caulking on HVAC ductwork were all negative, except for one sample analyzed by TEM which indicated <1% Chrysotile (i.e., trace materials).

Tan and black mastic sampled on foam insulation were negative for asbestos.

One homogeneous area having mudded elbows on the pipe was negative for asbestos.

The complete asbestos laboratory analytical report is provided in Appendix A, a photograph log is provided in Appendix B, and HRP inspector’s license documentation is provided in Appendix C.

5. RECOMMENDATIONS

Based on the findings of this pre-demolition/renovation asbestos survey, Thunderbird and HRP recommend the following:

All black mastic on fiberglass insulation throughout the crawl space should be assumed positive for asbestos.

All tan mastic on fiberglass insulation throughout the crawl space should be assumed positive for asbestos unless it has been tested and confirmed to be negative.

All black mastic on the duct insulation throughout the crawl space should be assumed positive for asbestos.

Black mastic used to seal pipe and conduit penetrations through the slab above the crawl space should be assumed positive for asbestos.

Assign appropriate response measures to all significantly damaged ACM insulation in the crawl space. A list of areas having significantly damaged ACM TSI confirmed during the survey is provided as follows. Any additional areas of damaged materials matching these descriptions that may not have been identified during the survey should be assumed as ACM until sampled and confirmed otherwise.

o Areas adjacent to all stairs and exterior points of access have damage from higher traffic areas. (Primarily black mastic and tan mastic on fiberglass pipe insulation)

Block A (Southeast Quadrant) o Area under the Boiler Plant, Column Row L to K and Column Row 5 to 7. (Primarily black mastic on fiberglass pipe insulation) o Area near Column Row K to M and Column Row 7 to 10. (Primarily black mastic and tan mastic on fiberglass pipe insulation) o Area near Column Row R to S and Column Row 11 to 12. (Primarily tan mastic on fiberglass pipe insulation) o Area near Column Row H to J and Column Row 15 to 16. (Primarily black mastic on fiberglass pipe insulation)

Block B (Northeast Quadrant) o Area near Column Row D to F and Column Row 8 to 9. (Primarily tan mastic on fiberglass pipe insulation) o Area near Column Row F to G and Column Row 15 to 16. (Primarily black mastic on fiberglass pipe insulation)

Block C (Southwest Quadrant) o Area near Column Row G to H and Column Row 24 to 26. (Primarily black mastic and tan mastic on fiberglass pipe and duct insulation)

Block D (Northwest Quadrant) o Area near Column Row E to F and Column Row 17 to 19. (Primarily black mastic on fiberglass pipe insulation) o Area near Column Row C to D and Column Row 24 to 25. (Primarily black mastic on fiberglass pipe insulation).

While EPA and SCDHEC do not specifically regulate trace materials, the federal Occupational Safety and Health Administration (OSHA) has established work practice requirements for disturbance and handling of these materials. As such, these requirements should be consulted prior to disturbance or handling of the red HVAC fire-stop material.

Dear Customer:

Enclosed are asbestos analysis results for PLM Bulk samples received at our laboratory on July 17, 2023. The samples were analyzed for asbestos using polarizing light microscopy (PLM) per the EPA 600 Method.

Sample results containing >1% asbestos are considered asbestos-containing materials (ACMs) per EPA regulatory requirements. The detection limit for the EPA 600 Method is <1% asbestos by weight as determined by visual estimation.

Thank you for your business and we look forward to continuing good relations.
