About this file

This is a Past Performance Questionnaire (PPQ) template for the 60th Contracting Squadron at Travis Air Force Base, California, regarding pavement rehabilitation services for Building 836 and Building 31 parking lots. The questionnaire is designed to collect performance ratings from recent customers of companies bidding on solicitation FA442726Q1150, which requires contractors to provide comprehensive pavement rehabilitation services including professional services, permits, equipment, labor, tools, materials, and ancillary items necessary to restore designated parking lot areas to acceptable operational condition and prevent further deterioration.

Respondents must complete and submit the PPQ as a PDF to A1C Trong Hoang, Contract Specialist, at trong.hoang@us.af.mil by 24 September 2026 at 10:00 AM PDT. The questionnaire requires detailed information about the offeror (the contracting company being evaluated), the respondent (the customer providing the rating), and the specific contract performed, including contract name, description, performance period, contract value, and whether work was performed as prime or subcontractor. Performance ratings are assessed on a scale of 1 to 5 across six categories: Quality of Service, On-Time Delivery of Service, Management of Service, Communication, Contract Terms and Conditions Adherence, and overall contractor performance. Respondents must also indicate whether Corrective Action Reports were issued and whether they would award future contracts to the contractor. The completed questionnaire becomes part of the official source selection records and may be verified by contacting the Contract Specialist at trong.hoang@us.af.mil or 707-424-7769.
