About this file

This document is a Contractor Responsibility Verification form based on FAR 9.103-9.104 standards that requires prospective contractors to demonstrate their qualifications for federal contract award.

The form mandates contractor submission of supporting documentation across six responsibility factors: (a) Financial Resources—requiring profit/loss statements, balance sheets, lines of credit, or written confirmation from a financial institution of adequate resources; (b) Meeting Delivery Requirements—requiring contractor certification of ability to meet proposed schedules considering existing commitments; (c) Performance Record—requiring minimum two past performance references from the last three years, capability statements for new entrants, or evidence of prior CPARS/SPRS ratings; (d) Business Integrity and Ethics—requiring certification that SAM.gov Responsibility/Qualification section information is current and accurate, including any exclusions or proceedings; (e) Organizational Structure—requiring confirmation of adequate experience, accounting controls, operational controls, and technical skills; and (f) Production Capabilities—requiring submission of technical equipment and facilities information or ability to obtain them. A company representative must certify under penalty of perjury that all information provided for factors (a) through (f) is correct and accurate. The form serves as the verification mechanism for determining contractor responsibility prior to federal contract award and applies to all business sizes, with special provisions noted for small business concerns.
