About this file
This is a Past Performance Questionnaire template designed to evaluate contractor performance on federal contracts, specifically for solicitation HM047626R0055 related to Terminal Instrument Procedures (TERPS) development for the National Geospatial-Intelligence Agency (NGA).
The questionnaire is completed by both the offeror and an evaluating organization to assess contractor performance across multiple dimensions. Offerors must provide contract details including contractor name, contract number, contract value, award date, completion date, program manager information, contract type, and a summary description including personnel turnover rates for cleared personnel. The form indicates whether classified work was involved and the classification level (Secret, Top Secret, or Sensitive Compartmented Information). The evaluating organization completes a separate section identifying the evaluator's name, rank, duty title, organization, and the observed performance period. Performance is rated on a scale from 0-10 (No Score through Outstanding) across three primary categories: Management Performance (Personnel and Program Management with 12 evaluation criteria), and Technical Performance (Technical Approach with 6 criteria and Quality Assurance with 2 criteria). Evaluators must provide detailed comments explaining ratings of Outstanding, Good, Marginal, or Unsatisfactory performance, and indicate whether unsatisfactory performance was previously communicated to the contractor and whether they would use the contractor again. This template serves as source selection information protected during the government's evaluation process for past performance assessment.
