About this file

This document is an Attachment 7 - Past Performance Information Sheet (PPIS) for Radiological Sample Analysis Services. The form provides detailed guidelines for contractors to submit past performance references, requiring specific information including a point of contact, contract details, service descriptions, and performance insights. Key requirements include identifying a single, external POC (not from the offeror's organization), providing contract numbers, dates of service, award value, performance locations, and a detailed explanation of services performed. Contractors must clearly demonstrate how their previous work correlates to the current solicitation's requirements, including describing their role, service similarities, and any problems encountered during previous contract performance. The document emphasizes the need for precise, specific information about the radiological sample analysis services previously provided.
