About this file

This document is an asbestos inspection report for the Louis A. Johnson VA Medical Center located in Clarksburg, West Virginia. Skelly and Loy conducted an inspection of areas affected by planned renovations to the Physical Therapy, Eye Clinic, and Food Preparation areas, finding asbestos-containing materials including pipe insulation debris containing 1.5% chrysotile asbestos, pipe insulation in cement containing 15% chrysotile and 10% amosite, air cell pipe insulation containing 25% chrysotile, and mudded pipe fitting insulation containing 20% chrysotile. The report provides inspection details, sampling methods and locations, laboratory analytical results, and photographs. It recommends that removal of any asbestos-containing materials disturbed by renovation be performed by a licensed West Virginia abatement contractor according to applicable standards.

The related federal contract opportunity is for construction services for a Nutrition and Food Services addition and Eye/PT Clinic addition at the Louis A. Johnson VA Medical Center in Clarksburg, West Virginia. The solicitation is issued by the Department of Veterans Affairs Technology Acquisition Center located in Austin, Texas.

REPORT

Office Locations: Harrisburg, PA Morgantown,WV State College, PA Hagerstown, MD Hunt Valley, MD

Mr. Peter Moser Project Manager Bancroft Architects and Engineers 700 Nicholas Blvd.

Elk Grove Village, IL 60007

RE: Limited Asbestos Inspection A/E Services for Nutrition and Food, Physical Therapy and Eye Clinic Addition Louis A. Johnson VA Medical Center 1 Medical Center Drive Clarksburg, West Virginia

Mr. Moser:

On September 23rd and 24th, 2021, Skelly and Loy, a Terracon Company, conducted a limited inspection for asbestos-containing material (ACM) in the Physical Therapy, Eye Clinic, and Food Preparation areas for the renovation project at the Louis A. Johnson VA Medical Center located at 1 Medical Center Drive in Clarksburg, West Virginia. The inspection was limited to areas which are to be affected by planned renovation activities. This inspection was completed in accordance with appropriate Federal and West Virginia asbestos regulations.

ASBESTOS INSPECTION

Appropriate safety and hygiene protocols were observed during all site activities. A visual inspection was conducted in all accessible areas within the proposed renovation area. Since a majority of the areas in the renovation areas were in use during the inspection activities, destructive measures for the collection of bulk samples were not used to gain access to inaccessible areas in occupied portions of the building; i.e. pipe chases, wall cavities, etc.

Discreet sampling methods were used in areas of concern during this inspection. Suspect asbestos building materials were classified into homogeneous areas (HAs), and bulk samples of each HA were collected based on the United States Environmental Protection Agency (US EPA) random sampling protocol.

Appropriate precautions were taken when sampling friable materials. The bulk sampling areas were first wetted with amended water and a small portion was then removed using a razor knife or other means. All bulk samples were sealed in plastic vials and logged with an appropriate chain of custody (COC) form.

Site asbestos inspection activities were conducted by Mr. Nathaniel Ruhl (WV inspector license #AI010737) and assisted by Mr. Ryan Steele. The inspector is USEPA trained and licensed by the West Virginia Department of Health and Human Resources. A copy of the inspector’s license is appended to this report.

Mr. Peter Moser

The visual inspection for suspect asbestos materials included areas above the drop ceiling tile.

Suspect ACM typically includes, but is not limited to, resilient flooring, mastics, ceiling tile, wallboard, building insulation, plaster, thermal system insulation (TSI), construction caulk, wall tar waterproofing, cementitious sheet board and pipe and ceramic tile grout.

Upon identification of a suspect material, a bulk sampling scheme was developed which included the collection of the appropriate number of bulk samples per homogeneous material. A homogeneous material is defined as a material that is uniform in color, texture, and age of installation. All bulk samples of suspect ACM found in the building were documented on COC forms with results being presented on the analytical reports.

During the inspection, the quantity of suspect ACM was also documented. Each suspect material was assessed as either friable or non-friable. By definition, friable material can be crumbled, pulverized, or reduced to powder by hand pressure. In addition, if non-friable material is disturbed or damaged, the material has the potential to become friable.

Asbestos bulk samples were analyzed using Polarized Light Microscopy (PLM) by US EPA Method 600/R-93/116. The bulk samples were shipped to Batta Laboratories (BATTA), located in Newark, Delaware, for analysis using PLM with dispersion staining, as specified by the US EPA. BATTA is accredited through the National Voluntary Laboratory Accreditation Program (NVLAP). By law, the laboratory analyst is required to analyze all layers of a submitted sample if multiple layers exist, unless otherwise directed in the COC. Based on the information gathered, the following summary of asbestos and non-asbestos findings and conclusions is presented.

Table 1.1, Summary of Asbestos Analytical Results, lists and characterizes all samples collected and analyzed from the Louis A. Johnson VA Medical Center. Bulk sample results are reported by percent and type of asbestos identified by laboratory analysis. Copies of the analytical reports and COC documentation are attached. Materials identified to contain greater than one percent asbestos are considered by definition to be ACM. Likewise, materials identified to contain asbestos equal to, or less than, one percent asbestos are considered by definition to be non-ACM. The location(s) of all ACM identified within the structure can be ascertained by cross-referencing the room numbers/descriptions listed in the “Location” column of the tables to the room number/descriptions designations on the enclosed figures.

· Of 44 suspect homogeneous materials sampled, 158 bulk samples (including layers) were collected and submitted to the laboratory to be analyzed.

· Of the sampled suspect materials, Pipe Insulation Debris (HA 29), Pipe Insulation in Cement Floor (HA 30), Air Cell Pipe Insulation (HA 34), and Mudded Pipe Fittings Insulation (HA 35) were identified as containing asbestos. All ACM is considered friable.

This report is accurate to the best knowledge of the inspectors. It must be reiterated that the asbestos inspection was limited to visible and accessible portions of the Louis A. Johnson Medical Center and that ACM not identified through this inspection may be present in inaccessible areas of the structure. Should suspect ACM beyond that identified through this inspection be encountered at any time in the future, Skelly and Loy recommends that it be treated as ACM until clarification by valid analytical methods is obtained. Skelly and Loy further recommends that all laws, regulations, and guidelines, governing waste disposal and the protection of human health and the environment be strictly adhered to when working with or near any confirmed or suspect ACM. The inspectors and Skelly and Loy assume no liability for financial or health consequences resulting from actions or lack of actions taken by clients as a result of this inspection. If ACM is to be disturbed by planned renovation activities, removal must be performed by a West Virginia licensed asbestos abatement contractor according to West Virginia Department of Veteran Affairs standards.

If you have any questions or comments concerning the contents or interpretation of this report, please call Mr. Daniel Davis at (412) 828-1412.

Sincerely, SKELLY and LOY
