About this file
This is an insurance requirements attachment for the State of Colorado Department of Personnel and Administration, applicable to contractors providing services to the state. The document establishes mandatory insurance coverage requirements that all contractors must obtain and maintain throughout the duration of their contracts. Contractors are required to submit proof of insurance, typically in the form of certificates on an ACORD form, within seven business days of the contract's effective date but before service commencement. Renewal certificates must be provided no later than 15 days prior to coverage expiration dates. Insurance policies may be issued as blanket policies covering recurring services, and only one certificate is needed to satisfy compliance for multiple active purchase orders or contracts with the Central Services Division.
The standard insurance requirements include Workers' Compensation Insurance and Employer's Liability Insurance for all employees; Commercial General Liability Insurance with minimum limits of $1,000,000 per occurrence and aggregate; Automobile Liability Insurance with $1,000,000 combined single limit for any auto; and Professional Liability Insurance when applicable to the specific procurement. The State of Colorado must be named as an additional insured on General Liability and Automobile Liability policies, with such coverage being primary over any state insurance programs. All policies must include provisions for 45 days' notice prior to cancellation or non-renewal, waive subrogation rights against the state, and be issued by state-approved insurance companies. Public entities subject to the Colorado Governmental Immunity Act must maintain only such liability insurance as necessary to meet liabilities under that act. The Purchasing and Contracts Administration office in Denver serves as the point of contact for all insurance submissions and inquiries.

Standard Insurance Certificate Requirements for Contractors
Providing Services to the State of Colorado (updated: March 2009 – 8 - 2011)
Instructions: Contractors hired to perform services for the State of Colorado are required to have appropriate insurance coverage in place for the term of the services. Prior to performing the services, the Contractor is required to provide proof of insurance coverage which usually is in the form of insurance liability coverage certificates and may be on an “ACORD” form, an insurance industry standard document. Insurance policies which do not contain restrictions to specific jobs or services may be used as a blanket policy to cover recurring services performed for the State of Colorado. Please note: policies which restrict the requirements below without advance and written State approval may be deemed insufficient and can be cause for cancellation of State issued purchase orders or contracts. Additional coverage, such as professional liability, may be required in specific procurements that have industry licensure or certifications. If you have more than one purchase order (PO) or contract with the Central Services Division of the Colorado Department of Personnel and Administration, only one copy of the certificate is needed to satisfy compliance for all active PO’s or contracts.
Make the Certificate out to and send certificates to the attention of the Purchasing/Contracts Office:
