About this file
This file is an RFQ Questions Template designed for submitting inquiries related to the Request for Quotations for Occupational Health Medical Director and Employee Assistance Program (EAP) Coordinator Support at John C. Stennis Space Center (Solicitation Number 80SSC026Q0008). The template is a standardized form with 19 numbered rows for submitting questions, requiring offerors to identify the RFQ Reference (attachment number, page number, and section number) and provide their specific question in the corresponding columns. Questions should be submitted in the order they appear in the RFQ, with the option to add additional rows as needed. The template includes a designation for "General" questions that do not pertain to specific RFQ sections.
This is a template document and does not contain substantive details about the solicitation requirements, evaluation criteria, or contractual terms. Offerors using this template should reference the actual RFQ and associated attachments for specific information regarding the Occupational Health Medical Director and EAP Coordinator support services required by NASA's Stennis Space Center, which is a total small business set-aside under NAICS Code 621111 with a $16M size standard.
